It was estimated that approximately one barrel of produced water was released when the blender tub overflowed onto containment. The produced water had left containment in at least two areas and impacted the well pad on opposite sides of the AST.
All free standing fluids were removed from the pad and the containment with a vacuum truck. Impacted materials on the well pad were excavated and taken off-site for disposal. The excavation activities were conducted with the use of soil conductivity probe readings to delineate the extent of impact and to determine the limits of excavation. Following the excavation of impacted materials from the well pad, confirmation samples were collected and new containment was installed in the affected areas. Approximately one barrel of brine was released due to a leak in the packing of one of the pumps on-site. The release of brine was completely contained and the standing fluid was removed from containment with a vacuum truck. There was no impact to the ground surface or to water. Mr. Sam McLaughlin from CNX reported the details to Mr. Benjamin Dillie from the Department. A site visit was conducted on June 2, 2016 with WQS Supervisor Mr. John Carson to review work completed on an earlier matter and to review the details of recent incidents. Approximately 15-20 gallons of hydraulic fluid was released from a failed hydraulic line on the blender pump. The fluid was released into secondary containment and did not impact the ground surface or water. The secondary containment was vacuumed out and the blender pump was repaired. The incident has been determined to be In Compliance with Policy (ICP). No violations are being cited at this time. |

I conducted a site visit the same day and spoke with Mr. Jessie Valentine and Mr. Kyle Borden from CNX Gas Company regarding the incident and the ongoing response.
A faulty O-ring seal in a hose connected to the blender failed which caused the release of brine from the line. It was estimated that approximately four barrels of brine were released due to the faulty seal. Approximately two barrels remained on containment while the remaining two barrels left containment and impacted the well pad. CNX had determined that the majority of brine that had leaked through damaged containment while a smaller portion had splashed over the berm surrounding the blender. I conducted a site visit on the same day to obtain additional information regarding the release and the response. An area of stained gravel was identified near some containment that was being staged on the well site. The stained gravel area was tested with a conductivity probe which identified elevated conductivity readings. There was no fracturing activities occurring on this area of the well pad so it was concluded that residue on the containment mixed with precipitation had impacted the well pad. The containment had not been used at the GH58 well site, although use of the containment at some other site could not be ruled out and would be the most likely explanation.
