2492546 Primary Facility 2016-06-06 Incident- Response to Accident or Event Violation(s) Noted On 6/16/2016, I performed an inspection of Rice Energy's Jacobs well pad site. Present at time of inspection was Jared Holman (Rice Company Man). The site is located in Gray Twp., Greene County. The inspection was performed as a incident inspection related to a reported brine spill by the operator.

The operator reported a spill of approx. 1 barrel of produced water off of containment to the well padon 6/3/16. I met w/ Jared Holman, the Rice Drilling Consultant on site to review the incident. Mr. Holman stated that Rice personnel had removed impacted material from the well pad for disposal. Cudd Energy Services where on site completing coil tubing operations on the Jacobs 11H well, when a produced fluid line had ruptured and discharged produced fluid to the secondary containment on site. Cudd did not report this spill to containment and then parked a truck on the containment berm causing the produced fluid to discharge on the well pad. 2498221 Primary Facility 2016-06-14 Follow-up Inspection Outstanding Violations - No Viols Req'd On 6/14/2016 I performed an inspection of Rice Energy's Jacobs Unit well site. The site is located in Gray Twp., Greene County. The inspection was performed as a routine inspection because of a requested minor modification to the ESCGP permit to allow for restoration of the site.

The Act 9 Site ID sign was in place and legible.

The operator reported a spill of approx. 1 barrel of produced water off of containment to the well pad on 6/3/16. Violations were noted on the 6/6/16 inspection related to the brine release. The operaor has notified the department upon removal of the secondary containment at the location of the first release, stained and impacted soils were noted under the secondary containment. The operator has begun characterizing the soil beneath the containment and has removed visually impacted soils.

2510477 Primary Facility 2016-08-10 Compliance Evaluation No Violations Noted On 8/10/2016 I performed an inspection of Rice Energy's Jacobs Unit well site. The site is located in Gray Twp., Greene County. The inspection was performed as a compliance evaluation inspection related to violations on site on 6/6/2016.

The Act 9 Site ID sign was in place and legible.

The operator reported a spill of approx. 1 barrel of produced water off of containment to the well pad on 6/3/16. Violations were noted on the 6/6/16 inspection related to the brine release. The operaor notified the department upon removal of the secondary containment at the location of the first release, stained and impacted soils were noted under the secondary containment. The operator has characterized the soil beneath the containment and has removed impacted soils.

The area has been regraded and stabilized with vegetative growth.

2788847 Primary Facility 2018-10-10 Routine/Complete Inspection No Violations Noted On October 10, 2018, the Department performed an inspection of the EQT/Rice Energy Jacobs well site. Present at time of inspection was Carey Bell and Shane Closkey (EQT). The site is located in Gray Twp., Greene Co. The inspection was routine inspection of the site related to a request by the operator to view the site, prior to the submission of a Notice of Termination of the ESCGP2 permit to the Department.

The operator has requested a meeting on site to review the condition of the site prior to submitting a Notice of Termination for the ESCGP2 permit.

The well site is stable with greater than 70% uniform perennial vegetative cover. The operator has reclaimed the sediment trap on site and has removed all the E&S BMPs. The operator has reclaimed the well pad to the interim restoration plan. The Department would recommend termination of the ESCGP2 permit given the condition of the site at time of inspection.

6 wells have been drilled to completion on the site and are equipped for production.
