The site is located in Aleppo Twp., Greene Co. The inspection was a routine inspection during setup for hydraulic fracturing activity to begin on site.

The operator has setup a Frac tank manifold system, in secondary containment, for storage of Frac fluid on site. The well pad secondary containment has been installed and the Frac spread was being setup at time of inspection. An above ground TWL has been installed to an AST on the Parry well pad and a water withdrawal from North Fork of Dunkard Fork will support the hydraulic fracturing operations at the Behm site.

Four wells have been drilled to total depth on site. The 8H well was in production, but has been temporarily shut-in for hydraulic fracturing operations on the 2H, 4H, and 6H wells.

2706094 Primary Facility 2018-03-01 Routine/Complete Inspection No Violations Noted On March 1, 2018, the Department performed an inspection of the EQT Charles Behm well pad. The site is located in Aleppo Twp., Greene Co. The inspection is CEI associated with violations against the operator for failing to terminate the ESCGP1 permit prior to the expiration of the ESCGP1 permit.

The operator has requested an inspection of the site prior to submitting a Notice of Termination to the Department for the expired ESCGP1 permit. The site has been restored to the interim restoration plan. The site is stable and vegetated with greater than 70% perennial vegetative cover.

The access road and well pad are in good condition.

4 wells have been drilled to completion and are equipped for production. The 10H well on site has been plugged and the well cellar is covered with a steel plate. The 8H well is listed as regulatory inactive status in the Departments records, but appears to have been placed back into production. The operator will need to send notification to the Department if the status on th

2766194 Primary Facility 2018-08-02 Routine/Complete Inspection No Violations Noted On August 2, 2018, the Department performed an inspection of the EQT Charles Behm well pad. The site is located in Aleppo Twp., Greene Co. The inspection is routine inspection related to a well site restoration report sent by the operator.

The site has been restored to the interim restoration plan. The site is stable and vegetated with greater than 70% perennial vegetative cover.

The access road and well pad are in good condition.
