2591943 Primary Facility 2017-05-02 Drilling/Alteration No Violations Noted Well has been drilled to total depth. Operator cemented production casing on 4/30/17. Operator set production casing at 13,914’ TMD / 7,627’ TVD.
2665926 Primary Facility 2017-11-28 Routine/Complete Inspection No Violations Noted Fracing 2H, 4H & 6H currently. At approx 15 or 16 of 37 zones on each well. Operator reports no anomalies or excess pressures. On site inspection showed no apparent violations. NOVIO
2666098 Primary Facility 2017-11-27 Incident- Response to Accident or Event Violation(s) Noted On November 27, 2017, the Department performed an inspection of the Rice/EQT John Briggs well pad site. Present at time of inspection was Mike Cromley (EQT). The site is located in Aleppo Twp., Greene County. The inspection was a incident inspection during hydraulic fracturing operations related to a reported reuse water release on site by the operator.

5 wells have been drilled to TD and are being completed on site. There are 3 wells on site that are already in production. The 8 wells on site remain permitted to Rice Energy and the permit transfers have not been processed by the Department. EQT is completing the wells on site as new owner of Rice Energy. The operator is currently hydraulically fracturing the 2H well. The operator reported a release of reuse water to the well pad of approximately 10 barrels after a steel line failure during hydraulic fracturing operations on 10/23/17. A sump has been excavated on the southeast corner of the pad near the recent release after initially excavating the well p

2708372 Primary Facility 2018-03-08 Routine/Complete Inspection No Violations Noted On March 8, 2018, the Department performed an inspection of the EQT/Rice Energy John Briggs B well pad site. Present during the inspection was Travor Coslett (Completion Consultant) and Dave Guidas (EQT). The site is located in Aleppo Twp., Greene Co. The inspection was CEI related to violations noted on 11/27/17 and 12/13/17.

The operator has coiling tubing operations active on site drilling out frac plugs on recently hydraulically fractured wells on the site. The coiling tubing operations are on on secondary containment which is in good condition. The operator has remediated the two reported reuse water releases on the well pad. The operator has recently completed construction of the PCSM structures on the site.

9 wells have been drilled to TD on the site, and proceeding towards placing the wells into production. The 9 completed wells have been drilled during return operations to the site.

2728821 Primary Facility 2018-05-11 Administrative/File Review No Violations Noted Completion report initial review completed.
