2633151 Primary Facility 2017-08-21 Compliance Evaluation No Violations Noted On August 21, 2017, the Department performed an inspection of the Rice Energy, Herold A well pad site. The site is located in Aleppo Twp., Greene Co. The inspection was a compliance evaluation inspection related to violations noted on July 24, 2017.

The operator begun drilling activity on site without having a pre-operational inspection and completion of construction of the site. The drill rig is currently on site completing the vertical section of five wells spud on the site. The operator has replaced failed E&S BMPs on site and has temporarily stabilized the unfinished well pad fill slopes and PCSM structures. All the construction equipment has been removed from the site and the operator plans to return to complete well site construction after the rig moves off site. The operator will need to perform maintenance on the secondary containment which has the potential to fail on the southwest side of the well pad.

The operator is currently drilling 5 wells on the site.

The site is located in Aleppo Twp., Greene Co. The inspection was a routine inspection after construction of the site and during drilling operations on the site.

The operator has begun drilling activity on site. The drill rig is currently on site completing the lateral sections of five wells spud on the site. The operator has recently spud two new wells on the site. The 9H and 11H. All the construction equipment has been removed from the site and the operator has completed construction of the site. The operator will need to perform maintenance on the secondary containment which has the potential to fail on the southwest side of the well pad.

7 wells have been spud on site and drilling operations are on going.

Present at time of inspection was Clay Gifford, Mladen Ruzic, and Eddie Garcia (EQT). The site is located in Aleppo Twp., Greene Co. The inspection was an incident inspection related to a reported freshwater release to the well pad.

The operator initially reported a 20 barrel brine release to the well pad on 7/2/18, then clarified the release to be freshwater. A phone call to David Lemasters (EQT) on 7/2/18 confirmed that the release was freshwater. The operator had permitted and constructed the modular aboveground storage structure (MASS) as reuse water MASS. The tank was initially filled with reuse water, but due to lack of truck access to the well pad, the MASS has been filled during hydraulic fracturing operations by a freshwater pipeline to the site. The tank has been flushed with freshwater since hydraulic fracturing operation has begun, with residual reuse water present. At the time of the inspection hy
