Well label present but not consistant with Department records. Department contacted Operator Rep Ron Jenkins by phone the day after inspection to recommend Operator ensure permit number on well head is correct. Mr. Jenkins stated he would check his records and follow up with Department. Mr Jenkins contacted Department by phone later that same day and stated permit number observed by Department was incorrect and that lease operator has placed the correct permit number on the well head. Two brine tanks on site appear structurally sound. A follow up inspection will be conducted at a later date.

2553848 Primary Facility 2017-01-11 Routine/Complete Inspection No Violations Noted At time of inspection, well drilled and appears equipped for production. According to records, drilling occurred 6/26/1980-7/02/1980. Well tag present, legible. Two on-site brine tanks appear sound.

Well site is permanently stabilized with >70% uniform and adequate vegetative growth. No accelerated erosion and/or sedimentation issues observed in relation to well site. Minor ruts developing in access road. Department recommends Operator continue to monitor and maintain access road as necessary in order to prevent any potential pollution events from occurring.
