Violations were issued for this reportable release under inspection record 2271151 (enf ID 311034) and subsequently corrected under inspection record 2401982.

At time of inspection, well appears to be equipped for production. Well tag present, legible. No brine tank on-site. Location of release has been backfilled and is restored and is permanently stabilized with >70% uniform vegetative growth. Well site stabilized with no accelerated erosion and/or sedimentation issues observed at this time. Access road currently stabilized; however Department recommends Operator add additional rock and/or another stabilization measure to entrance in order to prevent any potential pollution events from occurring. In accordance with the Spill Policy, 15 day initial report received by Department on 5/30/2014 notifying the Department that the Operator will be utilizing the Alternative Remediation Method f
