Department contacted operator rep Kirk Elkin by phone. Mr. Elkin states the lease operator noticed dead vegetation when visiting site. Mr. Elkin feels approximately 25 gal of brine was released through a crack in the bottom of the brine tank. Mr. Elkin states the brine tank is empty and not located in a dike. Mr. Elkin does not feel that waterways are affected. Also, Mr. Elkin states temporary BMP's will be placed on site as needed. Operator is In Compliance with Policy at this time. Violation(s) may be issued based on follow up inspection(s).

2764638 Primary Facility 2018-08-15 Incident- Response to Accident or Event Violation(s) Noted Follow up inspection to inspection record #2764457 where operator reported a release of approximately 20-25 gallons of brine.

At time of inspection no brine tank on site (photo #1). Department observes an area lacking vegetation starting near where brine tank was located and continuing down gradient from site (photo #2). This area is approximately 55 feet long and averages 5 feet wide. Hay bales have been placed at the down gradient end of the impacted area (photo #3). No dead/stressed vegetation observed beyond these hay bales. Department contacted operator rep Kirk Elkin by phone. Mr. Elkin states the brine tank was removed and the hay bales were installed this morning. Mr. Elkin states excavation will begin as soon as possible and that he will follow up with Department with remediation progress. Department recommends operator ensure the temporary BMP's used on site are approved for this HQ-CWF watershed. As per inspection record #2386577, a release of 8.5 gal of crude oil occurred at this site on 6-29-1

2765830 Primary Facility 2018-08-22 Administrative/File Review Outstanding Violations - Viols Req'd ADMIN inspection to inspection record #2764638.

Department contacted operator rep Kirk Elkin by phone to request an update on site remediation. Mr. Elkin states excavation of impacted soil was completed on Monday (8-20-18). Mr. Elkin states approximately 17 tons of impacted soil were removed and hauled to Ohio for disposal. Mr. Elkin also states the site has been seeded and mulched. Department recommends operator continue to monitor and maintain disturbed area until permanent stabilization is achieved. Violations issued on 8-15-18 remain outstanding. A follow up inspection will be conducted at a later date.

At time of inspection areas excavated on 8-20-18 (see inspection record #2765830) appear to be seeded and mulched and vegetation is beginning to grow (photo #1-2). Hay bales remain installed at down gradient end of impacted area (photo #3). No brine tank on site. Department recommends operator continue to monitor and maintain site to avoid any E and S issues. Operator appears to be in compliance with policy at this time. Violations issued on 8-15-18 have been corrected.
