2396676 Primary Facility 2015-08-03 Administrative/File Review In Compliance with Policy Admin inspt in response to release reported under inspection 2396649.Upon receipt of spill report, Dept contacted and spoke to Opr rep Drew Underwood via phone at 0847.Per Mr. Underwood, a release of 4.16 bbl brine water to ground from a BT occurred as a result of the valve leaking on BT.Mr. Underwood stated that WT noticed leak while reading meter on 7/31 at 1145.Release was subsequently reported to the SWRO at 1338.Well was shut in and BT emptied.After BT was emptied, it was moved from its location so excavation of contaminated soils could commence.Mr. Underwood does not know the last time a WT was at site at time of this call, but agreed to locate info and provide it to Dept.Mr. Underwood reports that no waterways have been impacted as a result of this release.A FUI will be conducted to assess any environmental damage and/or issue any associated viols as necessary.
2396850 Primary Facility 2015-08-03 Incident- Response to Accident or Event Violation(s) Noted On-site inspt conducted in response to inspt 2396676 for 7/31 reportable release of 4.16 bbl brine water.At time of inspt, Opr rep Jason Canose and contractors on-site.BT previously emptied, disconnected, and removed from its former location.Dept observed a vertical spilt under valve of BT, which Opr believes is cause of release.Contractors actively excavating contaminated materials, utilizing a conductivity meter and visual observation as guidance.Area of excavation currently confined to former location of BT.Two roll-off boxes on-site.Dept recommended that tempBMPs are installed as necessary in order to prevent any add’l pollution events from occurring.During excavation, it was discovered that crude oil had also been released.Dept requested an updated quantity of substances released be provided to the Dept. Violations are being issued under 78.54 for this reportable release of production fluids.
