2479099 Primary Facility 2016-04-15 Routine/Complete Inspection In Compliance with Policy At time of inspection, well drilled and appears equipped for production. According to records, drilling occurred 2/08/2007-2/13/2007. Well tag present, legible.

Well site permanently stabilized with >70% uniform and adequate vegetative growth. Access road appears shared. Access road stabilized at this time. No accelerated erosion and/or sedimentation issues observed during this inspection. Department observed moisture and small drips on brine tank which appears to be originating from the flange located near the top of this tank. Drips do not appear to have reached the ground at this time. No dead and/or stressed vegetation observed. While on-site, Department notified Operator representative Jason Canose via phone of the above noted observations. Mr. Canose stated that an Operator representative would immediately be sent to the location for the appropriate follow-up. Mr. Canose will provide the Department with an update when it is available. At 1157, Mr. Canose notified the Department that no detectable flu
