2390121 Primary Facility 2015-07-15 Administrative/File Review In Compliance with Policy Admin inspt conducted as follow-up to record 2390118 for the reportable release of 2.5 bbl brine.Upon receipt of spill report, Dept contacted Opr rep J.Canose via phone at 1605.According to Mr. Canose, the release resulted after a groundhog dug out the supporting soil/gravel around a rock which then caused the rock to protrude into the base of the brine tank.Per Mr. Canose, the impacted area appears to be localized to an approximate 6 ft radius.Excavation is to commence in the morning after the roll-off boxes are delivered.Well site located within a pasture.Mr. Canose believes the spill occurred w/in the last 2 wks as this is the last time that a well-tender was on-site to record the tank levels.The release was discovered on 7/15/15 at 1300 when the well-tender recorded a 3 in drop in the fluid measurements, equating to a loss of 2.5 bbl brine water.Tank was immediately drained and the spill reported to the SWRO at 1450.No waterways are reported to be affected.A FUI will be conducted to assess any envir impact and/or issue any associated viols.
2391107 Primary Facility 2015-07-16 Incident- Response to Accident or Event Violation(s) Noted Incident Response Insp to Opr reported release of +/-2.5 bbls brine water:Admin Insp #2390118. At time of insp, Opr Rep. J.Canose /contractors on site. Well tag present.Bt disconnected from prod equip, moved from orig location, in secondary containment. J.Canose thinks groundhog digging under bt shifted dirt around rock,caused stress fracture,resulted in release of brine on to ground. Dept observed crack in bottom of bt. J.Canose stated well tender was last at site 2 wks ago. Excavator on site; removing impacted soils (determined by conductivity mtr) into 2 roll off boxes. Temp BMP’s installed down gradient of excavation. J.Canose stated Opr would be using ACT 2 for remediation. Dept recommends Opr ensure site is temp stabilized w/in 4 days of cessation of disturbance. Dept also recommends Operator continue to monitor/ maintain temp BMP’s and ar to prevent any E&S issues. Opr intends to fence off area. Inspected with WQS S. Foulk.Violations: Operator discharged brine or other fluids on or into the ground or into waters of the Commonwealth.

Temporary BMP's installed downgradient of backfilled area. Securty fence around site and backfilled area. Department recommends Operator remove temporary BMP's upon permanent site stabilization. A follow up inspection will be conducted at a later date to ensure site is permanently stabilized.
