1816483 Primary Facility 2009-07-22 Site Restoration Violation(s) Noted Drilling completed 7/10/08. Well tag in place. Well is equipped for production. Access road stabilized. Inspection revealed the following deficiencies: 1) Failure to restore site within 9 months of completion of drilling. Site was seeded and mulched, but vegetation did not grow on the majority of the site, and is far from the 70% uniform, perennial, vegetation required. 2) Failure to minimize accelerated erosion, maintain E&S controls, and to stabilize site. Silt fence was installed but not maintained. A washed out rut was found going across site and into an adjacent stream. 3) Stream discharge of industrial waste. In addition to the washed out rut, there is also a diversion ditch installed around the up-slope side of the site, continues around the side of the site, and eventually discharges sediment to the adjacent stream. Silt fence was installed to avoid this problem, but was not maintained. Sediment could be seen at the discharge point as well as downstream. 4) Discharge of brine to the ground.
