---|---|---|---|---|---|
1804242 | Primary Facility | 2009-05-29 | Routine/Complete Inspection | No Violations Noted | Well site appears to be in production. The site is well vegetated and there is no accelerated erosion and sedimentation noted on the access road or well site. Operator should continue to monitor the access road and well site for erosion and sedimentation. |
1804578 | Primary Facility | 2009-06-11 | Routine/Complete Inspection | No Violations Noted | Site ID sign in place. E&S plan on site. |

1839374 | Primary Facility | 2009-10-27 | Routine/Complete Inspection | No Violations Noted | At the time of inspection, the well was in production. The site ID sign and E&S plan were available for review. Contractors were actively reclaiming the well pad. The filter fabric fence was installed and functioning correctly. No accelerated E&S issues were noted in relation to well site or access road. Reminder that once the site has been structurally reclaimed the area should be permanently fertilized, seeded, and mulched to minimize and eliminate erosion and sedimentation concerns. |
1908024 | Primary Facility | 2010-08-18 | Complaint Inspection | No Violations Noted | At the time of inspection, Dan Rice of PC was onsite. Well completed on 05/28/09. This inspection was a result of a complaint filed with dept on Aug 16, 2010 of runoff from the site onto private property. Upon inspection, observed the installation of two diversion ditches around well site that led to a culvert pipe below pad that drains through a wooded area into another existing culvert. Operator and dept agreed that additional measures to ensure runoff does not occur during rain events on nearby private properties should be completed. Operator plans to rock ditches (to reduce flow), install a sediment trap (to resist erosion), and if possible divert runoff away from area near residence to opposite side of well. At the time of inspection, there was no evidence of accelerated erosion off the well site on complainant¿s property. |
