No accelerated erosion or sedimentation issues observed in relation to well site or access road. Brine tanks(s) were in sound condition and were being pumped at time of inspection. The well site was adequately and uniformly restored. Evidence of a minor brine leak/spill (dead/discolored vegetation) was observed in the area surrounding the well head, but no leaks or discharges were detected at time of inspection, appears issue was addressed.
2031115	Primary Facility	2011-12-20	Incident- Response to Accident or Event	Pending	ADMIN INSPT- Incident notification received on 12/19/11 from Don Winebark, CNX Co of a reported release of coal bed methane (waste) water onto the ground due to a 'small' pipeline leak- specific amount could not be determined. Department followed up with Mr. Winebark on 12/19/11 whom stated that a leak was identified on a pipeline near the R&P Coal NT 113 CBM well. There was visible staining to the soil- operator immediately isolated the leak and then began remediated. At the time of call operator actively removing soils. It is not believe that release reached any water ways of the commonwealth. On 12/20/11, Department received an email that stated " [CNX has] repaired 2¿¿CBM water line near NT 113. [CNX has] removed affected soil placed in lined sealed containment box on location, backfilled and mulched area. I plan to come back in spring add more soil if necessary, seed, lime, and mulch. " Dept will conduct a follow up inspection in the spring to ensure adequate restoration is maintained and sutained till then insp result pending at this time.
