---|---|---|---|---|---|
1828213 | Primary Facility | 2009-09-10 | Drilling/Alteration | No Violations Noted | According to records, well permit issued on 4/17/09. Site pad completed. Two pits completed not aligned with plastic. Based on site conditions, no indication of IW issues at the site. Well sign and E&S plan located at entrance way of site pad. No E&S issues at the site. Well marked with survey marker. |
1849067 | Primary Facility | 2009-12-03 | Routine/Complete Inspection | No Violations Noted | At the time of inspection, the well was staked, not yet drilled. The site was properly signed with E&S plan available for review. Site appeared to be constructed following E&S plan. Pit dug,but unlined. Filter fabric fence was installed on the below gradient side of the pad. Noted various areas of E&S concerns which I am recommending be addressed promptly prior to drilling. First, there were notable areas of prior disturbance occurring from around drill pit and running down gradient towards silt fence. This concern can be addressed by installing a rocked lined interceptor ditch. Access road entrance is stable, but noted that access road to well site was muddy. I also observed some disturbance to the field beside the access road, that may needed to be reseeded. Well site was temporarily reseeded and some growth was observed. |
1855472 | Primary Facility | 2010-01-07 | Drilling/Alteration | No Violations Noted | According to records, well permit issued on 4/17/09. Site pad completed. Two pits completed with 20 ml plastic. Based on site conditions, no indication of IW issues at the site. Well sign and E&S plan located at entrance way of site pad. No E&S issues at the site. PC had rig set and was starting to drill the conductor pipe on 1/8/10. TSS filter was present. |
1856134 | Primary Facility | 2010-01-14 | Routine/Complete Inspection | No Violations Noted | Drilling rig located onsite and drilling was ongoing at the time of the inspection. Below grade pit located onsite and had more than two feet of lined freeboard. Observed no accelerated erosion & sedimentation with respect to the acess road. The operator should continue to monitor the erosion controls and the stability of the access road until well development and site restoration activities are completed. |
1856359 | Primary Facility | 2010-01-14 | Drilling/Alteration | No Violations Noted | According to records, well permit issued on 4/17/09. Site pad completed. Two pits completed with 20 ml plastic. Based on site conditions, no indication of IW issues at the site. Well sign and E&S plan located at entrance way of site pad. No E&S issues at the site. PC drilled well on 1/11/10 and hit open mine around 460¿. PC called Rick Hoover (DEP) and determined to abandoned this well and skid the rig. PC filled bore hole with 12 tons of gravel and cemented the remaining hole. PC moved rig 20 ft north of former well. Well set conductor pipe at 13 3/8¿ to 18¿ and second string at 9 5/8¿ to 514¿ with 240 sks class A. Mine shaft hit at 468 ft to 474 ft. Third string 7¿ pipe set at 1241¿ with 150 sks50/50 3% and 75 sks class A. Anticipated depth at 3835¿. |
1930551 | Primary Facility | 2010-11-22 | Site Restoration | No Violations Noted | At the time of inspection, well drilled and in production. Tag present. Rig skid from R Shaffer 5. Site adequately and uniformly restored with successful vegetative growth. No accelerated erosion or sedimentation issues observed. Access road stable. Brine tank sound. |
