1847467 Primary Facility 2009-11-23 Routine/Complete Inspection No Violations Noted At the time of inspection, the well was actively being drilled. The site was properly signed with E&S plan onsite available for review. The pit sustained a 2ft of freeboard and was structural ok, but it is recommended that measures to secure the pit liner are enhanced, as the liner did not appear to be suitably secured with either backfill or stakes. Noted that filter fabric fence was installed down gradient from site, but it is recommended the operator monitor all sediment BMP¿s to ensure proper functioning ability in order to prevent potential pollution events such as accelerated erosion or sedimentation issues. Also noted, that inspector Paul Wyrwas made a phone call to operator noting the relocation of drill pit, which was not changed on the E&S plan. Also noted the prior approval was given, with the exception that proper landowner consent was received, that the well may be established closer than 200ft to an existing building/ water supply.
1847593 Primary Facility 2009-12-04 Drilling/Alteration No Violations Noted At the time of inspection, no E&S or IW issues at the site. Well sign and E&S plan located at the entrance of site. Pit aligned with 20 mil plastic. No freeboard issues. Conductor pipe set at 37 ft ¿ 13 3/8 ¿pipe. Current depth at 220¿ Second stringer 9 5/8¿ pipe was set at 210 ft using 75 sks and basket at 84¿. Permit and sedimentary log present. Water not recorded. Sedimentary consisted of ss/sh alternation and coal seam. No voids or methane detected. Filter fence set around the site. Permit set for 4975 ft well.
1917839 Primary Facility 2010-09-22 Site Restoration No Violations Noted During this inspection, well properly tagged and in production. Drilling completed on 12/2/2009. Site restored with minimum 705 vegetative cover, although recommend that operator reseed to ensure that uniform coverage is achieved across the site. Access road appears stable. No accelerated erosion or sedimentation issues were noted in relation to well site or access road. Appears that brine tank may have a small seep or minor (< 5gals) release occurred during pumping of tank as discoloration along a section of bottom of tank and rock underlying tank is noted. Spoke with Dean Walters of Penneco on 9/28/10 at 930am regarding the possible concerns with the tank, operator had plans to look at tank and get back to me on findings. A follow up inspection will be conducted to ensure action has been taken and/or cleanup measures are performed as needed. Accompanied with WQS Dana Burk during inspection.
1917845 Primary Facility 2010-10-01 Follow-up Inspection No Violations Noted This inspection is being conducted as a follow-up to a possible concern with the structural integrity of brine tank. Brine tank appears empty. Side of tank and rock were slightly discolored. Spoke to landowner, who stated that operator pumped out brine tank earlier in the week and had plans to repair tank early next week.
1931748 Primary Facility 2010-12-02 Routine/Complete Inspection No Violations Noted At the time of inspection, no E&S or IW issues at the site. Well completed with 8¿ x 4¿ pipe and reduced to 2¿ production line. Line connected to totalize meter, separator (showing 300 psi), heater running and 100 bbl brine tank (<10 bbl fluid). No signs of leakage around well head. Well tag present
2295836 Primary Facility 2014-05-07 Routine/Complete Inspection No Violations Noted At the time of inspection, no E&S or IW issues at the site. No signs of leakage around well head. Well tag present. Gas well was in production.
