2253204 Primary Facility 2014-03-11 Incident- Response to Accident or Event Violation(s) Noted On-site inspt in response to admin inspt 2252002 for reportable release of approx 1 bbl mixture of crude oil/production water.Well-tender Gordon Bish on-site.According to Mr. Bish, wellhead split, spraying oil, brine water, "white water" out of crack.Occurred b/w 3/6/14 to morning of 3/10/14.Lease opr discovered on 3/10/14 at 1200.Production equipment disconnected from well.Currently excavating impacted soils, all visible oil removed.Preparing to re-locate brine tank to excavate soils underneath.Two roll-off boxes on-site, addit'l boxes to be brought on-site.As pond is located approx 125 ft down-gradient from site of release, Dept recommends sampling is conducted to ensure contaminants did not reach waters of the Commonwealth.Opr to follow Act 2 guidelines for site remediation.Drilling occurred 7/21/2010-7/27/2010.Well label present.Dept recommends Opr monitor/maintain access road as increased traffic due to clean-up efforts could potentially result in an accel E&S pollution event.Viols issued under 25 PA CODE 78.54, 78.57A & 58 PA CODE SWMA 301
