Access road and site ok. Inspection revealed the following deficiencies: 1) Operator failed to notify surface landowners whose water supply was within 1000 feet of the well location prior to drilling. Plat listed one water supply within 1000 feet of the well with all others on public water. An additional water supply (well) was identified through this inspection approximately 350 feet from the gas well location that was not identified on the plat. Other water supplies are reportedly within the 1000 feet. A thorough review of the area must be done to identify all water supplies within 1000 feet of the gas well. These findings must be shown on a revised plat. This revised plat must be provided to all surface landowners or water purveyors whose water supplies are within 1000 feet of the well location. Revised plat must also be submitted to the Department.
