Conductor pipe (13 3/8¿) set at 78¿ (sanded in place). Water protection (9 5/8¿) was set at 373 ft. and cemented with 135 sks of Class A. Plug was down at 03:20 hrs. with 8 bbls return. Water was recorded at 80 ft. (10 gpm) and 400 ft. (10 gpm). Intermediate casing (7¿) was set at 1,222 feet and cemented with 220 sks of Class A. Plug was down at 11/21/11 at 11:08 hrs. with 12 bbls of return. Coal was recorded at 177-183 feet and 400 ¿ 405 feet intervals. Phillips was currently drilling the production casing to anticipated depth of 4980 feet. |

Pit improperly constructed/lined--installed pit liner is reinforced with clamps and rope tied to rebar and trees above pit location; this type of installation does not ensure a structurally sound pit and induces stress to the liner hindering physical strength which can potentially lead to a tear. It appears that liquid contents of pit are above the 2ft freeboard level in some areas Based on current conditions department is currently recommending that operator promptly and properly dispose of pit contents in order to prevent a possible pollution event. |
