2797835 Primary Facility 2018-10-23 Routine/Partial Inspection No Violations Noted This is a well located within the 2500' buffer zone for a current gas migration. At the time of inspection, the well was producing. The tubing, production, and intermediate casing valves are all open and producing. The pressure reading for the tubing was 16.1 psi, and the production casing pressure was also 16.1 psi. The intermediate casing was reading 100% methane, but no pressure was measurable.

The well was properly labeled. The well was equipped with a separator and 100 bbl plastic tank (which is contained within an earthen dike). Production reported in 2017 was 3605 mcf for 365 days.

2812108 Primary Facility 2018-12-07 Routine/Partial Inspection No Violations Noted At the time of inspection, well was being vented to a tank as requested by the Department. All valves were open, but the production casing is only opened about 1/2 way. This was so back pressure can be held on the well to prevent water intake from formation. Gas can be hard venting from the tank to atmosphere.
2818550 Primary Facility 2018-12-19 Routine/Partial Inspection No Violations Noted At the time of inspection, well is back online. The tubing valve is wide open and production casing is punched open. The annulus to production casing is being vented to atmosphere.
2824765 Primary Facility 2019-01-03 Routine/Partial Inspection No Violations Noted At the time of inspection, well was producing. The annuli remains vented. Inspection was done with Jeff Forsythe (a Diversified employee) and 2 other DEP employees. Gas samples were taken from the 7"-4 1/2", 4 1/2"-tubing, and inside of the tubing. Plan was to leave the annulus vented and permit production to continue for the other casings.
