I entered the location at 15:17 and began the inspection. The Poor Shot Transfer pad off Stony Gap Road, the Poor Shot Unit 2H well pad, The Poor Shot Unit 3H well pad, and the Poor Shot 1H well pad were all constructed under one ESCGP (ESX10-081-0023). The Poor Shot Freshwater Impoundment was never constructed.

A : At the Poor Shot transfer pad, the pad location remains at its originally constructed size. The Site Restoration Plan calls for Sediment Ditch #1, Sediment Trap #1, and Sediment Ditch #2 to be removed. In addition, erosion is noted on the north side of the pad near the western edge of Diversion Ditch #1. I recommend correcting this erosion as soon as possible. This part of the ESCGP has not been restored as per the ESCGP. B : At the Poor Shot Unit 2H well pad, the Poor Shot East Unit 1H (081-20092) and Poor Shot E Unit 2H (081-20278) wells were both in production. The Site Restoration Plan calls for one 60’ Infiltration Berm to be constructed. The Infiltr

2841891 Primary Facility 2019-02-11 Routine/Complete Inspection No Violations Noted On February 11, 2019 Department representative Jared Jacobini conducted a routine site inspection at the EXCO Poor Shot Unit 1H OG Well. The site sign, drilling permits, weekly E&S logbook, and emergency response plan were onsite for review.   PSCM's have not been installed. Minor erosion issues were along the southern border of the pad. The site is well vegetated meeting the 70% uniform, perennial, vegetative cover. Two wells were in production phase. One GPU was present and not within secondary containment (Photo#4). No leaks detected from production equipment.

The Department recommends addressing the following:

1. Production water tank secondary containment liner tears (Photo #1, 2).

2. Stabilizing the minor erosion observed along the edge of the pad.

3. Removing gravel from diversion ditch. (Photo #3).

4. Restore site per site restoration plan described in the ESCGP if the site will not be utilized.
