2744348 Primary Facility 2018-06-22 Routine/Complete Inspection No Violations Noted Weather was 66° and cloudy with drizzle at time of inspection. There were no ARD representatives on location at time of inspection. Well had one annulus vent that was plumbed to surface. Well had digital pressure monitoring equipment on the annulus vent, the production casing and the tubing that did not have a visible read out. There was a 0-5k PSI analog pressure gauge on the pay line that read approximately 200 psi at time of inspection. There was also a 0-10k PSI analog pressure gauge on the production casing that read approximately 0 psi at time of inspection. 0% combustible gas was detected in any area around the well at time of inspection. No violations were noted at time of inspection.
2793481 Primary Facility 2018-10-22 Routine/Complete Inspection No Violations Noted On October 22, 2018, Department representative Brad Kline conducted a routine site inspection at ARD Operating LLC. COP Tract Pad C only. No E&S issues were observed during the time of inspection. The site is stable and well vegetated exceeding the 70% uniform, perennial, vegetative cover.

There are a total of 4 producing wells, 1 regulatory inactive status well, and 1 plugged well located on this well pad. All wells are tagged with API/well permit numbers. No bubbling within the cellars was observed. There are 4 GPU’s and 4 steel vertical production fluids tanks onsite. Both GPUs and production fluid tanks were stationed in secondary containment.

No violations noted
