I entered the site at 10:12. At the time of the inspection, the Kenneth Schriner A 2H (081-20461), 4H (081-20457), and 6H (081-20458) were in production. ESX11-081-0125 expires on 2/8/2018. The well site was permitted with the Warrensville West Gathering Pipeline. Currently, the site remains at the originally permitted size, has not been restored or reduced, and no PCSM (Post Construction Stormwater Management) has been implemented on the site.

The most recent update to the well site restoration assessment Anadarko provided on 12/22/2016 indicates the calculations are complete and the site is in drafting and engineer review. At this time, the Department does not have a permit or permit application for the site. I recommend re-permitting the site (as planned) through a major modification or a separate permit to comply with PCSM requirements and implementing that plan including well site restoration. No Violations Noted.

2763081 Primary Facility 2018-08-07 Routine/Complete Inspection No Violations Noted Notice of Termination Inspection; 8/7/2018.

At the time of the inspection, 3 wells were in production and no one else was onsite. Post Construction Stormwater Management (PCSM) is accomplished with the use of a vegetative filter strip which remains undisturbed. Overall, the site fits the Site Restoration plans and can be considered restored. I recommend approving the Notice of Termination and I recommend the operator submit the Well Site Restoration Report as soon as possible. No violations noted.
