I entered the location at 12:58 and began the inspection. At the time of the inspection, three wells (081-20467, 081-20464, and 081-20462) were in production on the site. The well site was in good condition and no E&S issues were observed. The site has no active well permits that have not been drilled.

ESX11-081-0094 expired on 9/17/2016 without being terminated and the site has not been restored. The wells onsite were drilled in 2011 and the well site has not been restored. Anadarko’s most recent restoration timeline submission indicates PCSM will be permitted in 2016 and installed in 2017.

The Department considers a well site to be restored under Section 3216(c) of the 2012 Oil and Gas Act when the oil or gas owners or operators meet the following:

1. All permanent post construction stormwater control features as   the Post Construction Stormwater Management/Site Restoration Plan are in place.

2. Remaining impervious areas are minimized. See Section 102.8 (b)(4). Im

2643979 Primary Facility 2017-10-04 Routine/Complete Inspection No Violations Noted Weather was 64° and sunny at time of inspection. 5 1/2 x 9 5/8 annulus vent was to plumbed surface with an analog gauge attached that had a reading of approximately 50 psi at time of inspection. No other annular vents were observed at time of inspection. Well head had a 0 to 10,000 PSI gauge attached above the swab valve that red approximately 1000 psi at time of inspection.
