2325602 Primary Facility 2014-11-25 Incident- Response to Accident or Event Violation(s) Noted On 11/22/2014 Mark Barbier (Anadarko) reported approximately 20 barrels of produced fluid and associated production gas was discharged (uncontrolled to the atmosphere) from the 4H well due to a pipe joint failure on the wellhead. According to the call, the well was shut in manually by Anadarko personnel.
2521467 Primary Facility 2016-09-15 Routine/Complete Inspection No Violations Noted Routine Inspection; 9/15/2016.

I entered the location at 13:40 and began the inspection. At the time of the inspection, five producing wells (081-21014, 081-20562, 081-21015, 081-20563, and 081-21016) and one plugged well were located onsite. The well site was in good condition and no E&S issues were observed. The site has no active well permits that have not been drilled.

ESX10-081-0041 previously expired and ESG29-081-16-0003 is currently active. The wells onsite were drilled from 2011 to 2012 and the well site has not been restored. Anadarko’s most recent restoration timeline submission indicates PCSM will be permitted in 2016 (as noted above) and installed in 2017.

The Department considers a well site to be restored under Section 3216(c) of the 2012 Oil and Gas Act when the oil or gas owners or operators meet the following:

1. All permanent post construction stormwater control features as the Post Construction Stormwater Management/Site Restoration Plan are in place.

2. Remaining impervious are

2575494 Primary Facility 2017-03-21 Routine/Complete Inspection No Violations Noted Routine inspection of unvonventional well pad in production. A-4H well is in production with no detectable methane emissions. Bubbling noted on location, later discovered to be a phenomenon of the thawing ground. Methane emissions not detected by GES during sampling of location.
2578513 Primary Facility 2017-03-23 Routine/Complete Inspection No Violations Noted Routine inspection of unconventional well in production. No violations noted at time of inspection.
2581628 Primary Facility 2017-03-28 Follow-up Inspection No Violations Noted Follow up inspection of an unconventional well pad in production. Some bubbling still noted on the pad. 0% combustible gas emissions detected at time of inspection. No violations noted at time of inspection.
2587701 Primary Facility 2017-04-12 Follow-up Inspection No Violations Noted Follow up inspection of location and wells following discovery of a bubbling phenomenon on location. Phenomenon appears to have subsided, No methane detected during inspection. No violations noted at time of inspection.
2592384 Primary Facility 2017-05-03 Routine/Complete Inspection Violation(s) Noted Inspection of 11 3/4" x 8 5/8" annulus vent on well. Methane detected at time of inspection. Cited defective cement.
2643971 Primary Facility 2017-10-04 Routine/Complete Inspection No Violations Noted Weather was 57° and sunny at time of inspection. 5 1/2 x 9 5/8 annulus is plumbed to the surface and has a digital gauge attached. Gauge read 137 PSI at time of inspection. The second annular event is plumbed to the surface with an analog 0 to 1000 psi gauge attached. Gauge had a reading of zero psi at time of inspection. Well had had a digital pressure gauge attached above the swab valve that read 1104 PSI.
2687251 Primary Facility 2018-01-22 Incident- Response to Accident or Event Violation(s) Noted Spill response inspection; 1/22/2018.

Mark Barbier (ARD Operating) called the Department at 18:01 on 1/20/2018 to report the release of 176 gallons of a 30% methanol/water solution. The methanol/water mixture was released from a failed ball valve on the 4H(081-20563). 250 gallons of the released methanol/water mixture and melt water was vacuumed from the 3H well cellar later that night.

At approximately 13:45 on 1/22/2018, I met onsite with Mark Barbier and Rick Hall (both ARD). They showed me the failed valve leading to the 4H well and the excavated flow path into the 3H cellar. Material from the flow path across several feet of the pad surface had been excavated and placed in a overpack drum for disposal. The material under the excavation was frozen solid. I was only able to drive a digging bar 1/4 inch into the frozen surface. Water was streaming into the well cellars from the well pad surface due to warming temperatures and runoff. I was told the cellar would continue to be vacuumed. The conditio

2844657 Primary Facility 2019-02-26 Routine/Complete Inspection No Violations Noted Five wells staged for production. One well plugged. No free standing liquid on a day temperatures were below freezing. No leaks.
