There were no ARD representatives on location at time of inspection. Well had digital pressure monitoring equipment attached to the tubing, the production casing and the Intermediate x production casing annulus vent. The monitoring equipment did not have a readout. There were analog pressure gauges attached to the production tubing, production casing and the annulus vent. The 0-10k psi gauge on the production tubing read approximately 0 psi, the 0-6k psi gauge on the casing read approximately 1850 psi and the 0-6k gauge on the annulus vent read 0 psi at time of inspection. No violations were noted at time of inspection.
2790529 Primary Facility 2018-10-17 Routine/Complete Inspection No Violations Noted On October 17, 2018, Department representative Brad Kline conducted a routine site inspection at COP Tract 356 Pad. Monthly tank maintenance log, and weekly E&S inspection log were onsite for review. No E&S issues were observed during the time of inspection. The site is stable and well vegetated exceeding the 70% uniform, perennial, vegetative cover.

There are a total of 3 producing wells located on this well pad. All wells are tagged with API/we’ll permit numbers. No bubbling within the cellars was observed. There are 3 GPU’s within secondary containment and 1 sand separator.
