Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments

2471665 Primary Facility 2016-04-04 Routine/Complete Inspection No Violations Noted The well site remains at its originally permitted size and none of the 6 wells permitted wells on the site have ever been drilled. Six well cellars have been installed and containment remains installed around the cellars. Scrap pieces of containment and trash were piled at the northeast corner of the well pad. Full rolls of containment, pipe, barriers, and berms were also being stored onsite. Please remove trash being stored onsite. Erosion was noted on the southeast fill slope of the well pad. In addition, the topsoil pile lacks vegetative cover and has exposed topsoil. I recommend repairing the erosion/scouring on the fill slope and installing permitted PCSM (Post Construction Stormwater Management) features before the ESCGP expires.
2497559 Primary Facility 2016-06-30 Routine/Complete Inspection No Violations Noted Routine Inspection; 6/30/2016.

I entered the site off Logue Hill Road at 13:15 and met with Doug Byers (Atlas). The well pad is in the process of being reclaimed. Remaining E&S controls and a vegetative filter strip have been maintained from the previous pad construction. Additional silt sock has also been added. Overall, the site has been returned to original contours and the topsoil is being applied to the slopes. The site is in good condition and no E&S issues were noted. No Violations Noted.
