2707845 Primary Facility 2018-03-15 Routine/Complete Inspection No Violations Noted A routine inspection of this unconventional well pad was conducted March 15, 2018. The purpose of inspection was to verify the 1-H is plugged because no plugging certificate was found and the well currently holds an Active status. At the time of inspection, the 1H appeared to be a plugged 20" conductor only, with a plate welded across the top and a 2" candy cane vent as a plugged well monument with marker. No obvious issues were observed. There were 3 producing wells on the pad. I conducted a visual inspection of these three producing wells and did not discover any obvious issues.

Please submit a plugging certificate for the 1-H if it is in fact permanently plugged at your convenience.
