2465521 Primary Facility 2016-03-16 Routine/Complete Inspection No Violations Noted At the time of the inspection, eight wells were in production. The well site is predominantly vegetated but erosion and was noted on the topsoil stockpile and the area lacks adequate vegetation to prevent accelerated erosion. In addition, the well site remains at the originally constructed size and has not been restored as indicated in the permitted plans. The requirement under Section 3216 (c) of the Oil and Gas Act requires that "within nine months after completion of drilling of a well, the owner or operator shall restore the well site." The approved plans call for a portion of the well site to be scarified and production facilities are installed where the site is to be scarified. I recommend re-permitting the site to incorporate a feasible restoration of the site. No Violations Noted
2583444 Primary Facility 2017-04-13 Routine/Complete Inspection No Violations Noted Arrived @ 13:00. Weather: clear, light breeze. There were 8 wells capable of production on location. The wells were in two linear arrays: 5 closely spaced wells in one array, and 3 more spaced out wells in the second array. There were no personnel were onsite at the time of my inspection. For all eight wells, there was water in the cellars but no bubbling. The 9-5/8” x 5-1/2” vent was shut to a gauge. No other annular vents were visible. All pressures readings are digital.

Plants Evergreen A207H: Top gauge: 1000 psi, 9-5/8” x 5-1/2”: 204 psi, foam in use. For plumbing leading to the GPU, there were signs of small leaks on either side of the pressure valve. These signs were: rust at the bottom of the fitting, salt crystals at the lower portion of the joint, and rusty discoloration on the pad below. Plants Evergreen A208H: Top gauge: 969 psi, 9-5/8” x 5-1/2”: 3 psi, Plants Evergreen A209H: Could not read top gauge, 9-5/8” x 5-1/2”: 72 psi. Plants Evergreen A2H: Top gauge: 0 psi, 0 9-5/8” x 5-1/2”: 0 psi, well

2755619 Primary Facility 2018-07-13 Routine/Complete Inspection No Violations Noted Notice of Termination Inspection; 7/13/2018.

The wells onsite are in production and workers leaving the site said they had just finished painting the wellheads. The site is permanently stabilized and Post Construction Stormwater Management has been implemented using the natural vegetated filter strip. I recommend approving the NOT request and I recommend approving the Well Site Restoration Report when it is submitted.
