2438660 Primary Facility 2015-12-31 Complaint Inspection Violation(s) Noted The well had two annular vents. They were not labeled and both were 2”diameter. The vents were later identified by EXCO following the inspection. The 9-5/8” x 5-1/2” vent pipe had a 0-5000 psi analog gauge in place and the vent was shut-in. Venting of gas was observed when the vent was opened. The 13-3/8” x 9-5/8” annulus vent was open at the time of the inspection. Both vents were monitored with an Altair 5X gas meter calibrated on 12/28/15.
2438664 Primary Facility 2016-01-04 Complaint Inspection Outstanding Violations - No Viols Req'd Arrived @ 12:50. Weather: cold, windy, sunny with snow on the ground. The single gas well on location was locked and shut in. The cellar was filled with gravel and there was not a gauge on the top of the well head. There was no well tag. There were some quick ties and torn paper and plastic suggesting that the tag had degraded and not been replaced. The identity of the well was clear as it was the only one on location.

The well had two annular vents on the same side of the well. The 13-3/8” x 9-5/8” and 9-5/8” x 5-1/2” annular vents were open. Both vents were monitored with an Altair 5X gas meter calibrated on 01/04/2016. 13-3/8” x 9-5/8” – 27% methane, 9-5/8” x 5-1/2” - 38 % methane.

The Chaapel Hollow 6H is the only well on location. The purpose of this inspection was to check for methane gas on the well’s annular vents. 5-1/2” casing: 6,200 psi

Both vents were monitored with an Altair 5X gas meter calibrated on 04/12/16.

2483446 Primary Facility 2016-04-12 Routine/Complete Inspection Outstanding Violations - No Viols Req'd Arrived @ 10:15.Weather: cloudy, cold and windy, threat of rain. The purpose of this inspection is to observe pressure testing of the well head seals. Met with Bird Smith (EXC)). Jake Harriger and Bob DeJohn (Cameron) were testing the seals between the 9-5/8” casing spool and the 5-1/2” spool. The seals held 8000 psi for ¿ 10 minutes. This was 800 psi above the well head pressure of ¿ 6200 psi. Prior to my arrival they had packed out the seal to 12,000 psi, because there had been pressure in the space between the 5-1/2” and the flange outer diameter. The set of seals immediately below the 5-1/2” spool and the 9-5/8” spool tested good at 7800 psi. Prior to my arrival they had tested the seals between the A-section and the 9-5/8” spool and these had held at 2000 psi. At both annular vents there was an intermittent bubbling sound.

The result of this inspection is, “OUTNO” – Outstanding violations, no new violations, because violations for defective cement were cited on 12/31/15 (IR# 2438660). Left location @

2515030 Primary Facility 2016-08-29 Routine/Complete Inspection Outstanding Violations - No Viols Req'd Arrived @ 8:45. Weather: cloudy, breezy, but still hot and humid. The purpose of this inspection was to document EXCO’s mechanical integrity testing (MIT) at the Chappel Hollow 6H, a completed well which is shut in. I met with Bird Smith and Shane Lee (EXCO). Also onsite were Dave with a Multichem water truck whose purpose was to pump water down the annuli, and later a vac truck arrived to empty water from the cellar.

The MIT was initially conducted by pumping water into the 13-3/8” x 9-5/8” annulus. Pressures at the 13-3/8” x 9-5/8” and 9-5/8” x 5-1/2” annuli were monitored directly onsite with Crystal digital pressure gauges. Pressures were also recorded over the duration of the test with Spartex Wellhead Recorders (SS6100, calibrated 01/2014). Previous PBT results indicate that there is communication between the two annuli. The purpose of the MIT test initiated on 08/29 was to determine the integrity of the surface casing. 2536585 Primary Facility 2016-11-14 Complaint Inspection Outstanding Violations - No Viols Req'd Arrived @ 10:00, weather sunny and calm. This inspection was in response to a complaint (# 323549) alleging odor at the well pad because gas was being odorized, compressed, and trucked off location. Met with Bird Smith (EXCO) and Justin Kinney (contractor). The process was ongoing at the time of my inspection. In addition to the well head and GPU, there was a compressor, and ethyl mercaptan on site. An 18-wheeler containing a gas pressure vessel(s) was parked adjacent to the compressor. Pressure at well head, 6070 psi, was being choked down to 1150 psi, flowed through the GPU onsite, odorized, and then compressed. An ethyl mercaptan odor was prevalent at the well head. Operations began on October 14. No new violations were noted at the time of this inspection. The result was, “OUTNO” – Outstanding violations, no new violations, because violations for defective cement were cited on 12/31/15 (IR# 2438660).

2557071 Primary Facility 2017-01-26 Routine/Complete Inspection No Violations Noted Routine Inspection; 1/26/2017.

I entered the location at 09:05 and began the inspection. At the time of the inspection, the Chaapel Hollow Unit 1H (081-21201) was being produced by flowing gas directly into a tube trailer. Erosion was noted along the access road. The hillside had slumped in several large chunks. This area is currently frozen, but I recommend keeping an eye on this area when the snow melts. The well site is not yet required to be restored because of the active well permit but the same permit has been renewed since 2014 without action. No Violations Noted.

2583064 Primary Facility 2017-04-07 Follow-up Inspection No Violations Noted Follow-up Inspection; 4/7/2017.

I entered the site at 11:50 to follow-up on my previous inspection on 1/26/2017. Slumping of the cut slope along the access road has expanded since the previous inspection but does not appear to be causing further erosion or discharge of sediment. The Chaapel Hollow 6H (081-21201) has been producing while the Chaapel Hollow 7H permit expired on 3/28/2017 having been Proposed But Never Materialized. The tank pad remains constructed and the site has not been restored according to the plans. According to our permitting section, the practice or theory of managing stormwater runoff volumes by infiltrating into compacted pad surface/subsurface area is not considered a viable approach. I recommend addressing the erosion along the access road, re-permitting with proper PCSM,and restoring the site. No Violations Noted at this time.

2617399 Primary Facility 2017-07-19 Complaint Inspection No Violations Noted Complaint Inspection; 7/19/2017.

Rebecca Renner and l met with the complainant at 09:00. The complainant had several concerns including the condition of the access road, alleged water coming from the well, production practices, and failure to restore the well site. At the time of the inspection, only the production unit/ separator and were onsite. Equipment previously onsite for compression and trucking the gas offsite has been removed.

At the entrance to the (site off Rose Valley Road), the asphalt is cracking and slumping down the slope (see photos). Concrete barriers have been installed in this area as it is a steep slope with a dangerous drop (see photos). After discussing this with our engineers, they also suggested evaluating the drop inlet and attached cross pipe as water from this pipe could be saturating the hillside and making matters worse. The driving surface of the road is eroding in many locations. The erosion is causing water to flow down the road and bypass designed drainage. Erosion

2645502 Primary Facility 2017-10-10 Routine/Complete Inspection Outstanding Violations - Viols Req'd Arrived at 13:38. Weather: sunny & warm. Routine inspection, no personnel on site. One well on site, Chaapel Hollow 6H, not in production.

Too gauge: 6345 psi (digital) 5-1/2": 6064 psi (digital) 2 open annuli 9-5/8" x 5-1/2": annulus open to an open needle valve, full of water. Quarter-inch crust, appears to be mud water mix, along edge of opening. Combustible gas (>100 % LEL) bubbled up through fluid. Approximately 3 sets of bubbles every 5 seconds for about 30 seconds. This bubbling was intermittent. 13-3/8" x 9-5/8": open, filled with water, no bubbling. Left at 14:25. No violations were observed during this inspection, but previous violations were cited. For this reason, the result of this inspection should be, OUTST. As this is not an option with the mobile app, the result of this inspection is, PENDING, which will be changed in eFACTS to OUTST. (iIR changed on 2.12.18 - BAR)
