The interim RCE (Rock Construction Entrance) was installed behind the two houses onsite as discussed so mud was removed before entering residential driveways. The well pad was accessed via an existing road on the property as designed in the permitted plans. The well pad area has been cleared and all timber and tops have been stacked. According to the operator, the timber will be hauled when the access road is constructed to the timber piles. The majority of the access road has been cleared and a skid trail has been rough graded down the access road. No signs of accelerated erosion were noted onsite. I expect perimeter E&S controls including silt sock will be installed relatively soon before bulk earth disturbance begins. No Violations Noted.
2487635 Primary Facility 2016-05-25 Routine/Complete Inspection No Violations Noted Routine Inspection; 5/25/2016.

I entered the site off Sugarcamp Road at 15:20. The interim RCE (Rock Construction Entrance) remains installed behind the two houses onsite as discussed so mud was removed before entering residential driveways. I accessed the well pad via skidding road on the cleared access road route. The main construction has not been initiated and the timber remains stacked. No signs of accelerated erosion were noted onsite. I expect perimeter E&S controls including silt sock will be installed before bulk earth disturbance begins. No Violations Noted.

2504642 Primary Facility 2016-07-26 Routine/Complete Inspection No Violations Noted I entered the site off Sugarcamp Road at 08:51. The Rock Construction Entrance remains installed behind the two houses onsite as discussed so mud is removed before entering residential driveways. Perimeter E&S controls have all been installed and the majority of the access road area has been temporarily stabilized. At the time of the inspection tree tops and stumps were being ground into tractor trailers and hauled offsite on the well pad. Clearing for the infiltration berms is underway and installation of the infiltration berms has begun. No E&S issues were noted in spite of the rainfall last evening throughout the area. This inspection fulfills the Pre-Operational Inspection requirement of the O&G Act as amended by Act 13. No violations noted.
2570800 Primary Facility 2017-03-09 Preoperation Inspection No Violations Noted Routine Inspection; 3/9/2017.

I entered the site off Sugar Camp Road and met with Kirk Gregory (Seneca) at 13:00. We met for a pre-operational inspection of the site. At the time of the inspection, contractors were welding and installing flow-lines while the construction contractor (HRI) was onsite adding limestone to the pad surface. The flow-lines are should be installed and the pad surface completed in the near future. Welded steel cellars set in a concrete pad have been installed. The majority of the access road is well vegetated but a few slumps were fixed with erosion control blanketing. The majority of the slopes on the well pad have been stabilized with erosion control blanketing since much of the earthmoving was finished outside the growing season. Seneca plans to seed and mulch (most likely hydro-seed) in the growing season when conditions are appropriate. Overall, the site is in good condition. This inspection fulfills the pre-operational inspection requirement of the O&G Act as amended by

Operations were well organized, very clean. Left location @ 11:02.
2606266 Primary Facility 2017-06-15 Routine/Complete Inspection No Violations Noted Routine complete inspection; 6/15/1017. The Patterson rig 339 was on the Easton 30H (081-21634) running 9 5/8" intermediate casing in the hole. I walked the site with Justin Greene (company man). The containment looked excellent, clean, and well maintained. Cuttings management looks clean and well kept. Chemical storage was all on containment and clearly separated. Overall, the site looks very good. No violations noted.
2645939 Primary Facility 2017-10-11 Drilling/Alteration No Violations Noted Arrived at 11:00. Weather: warm, rain. Met with company man Anthony Murphy. Keane Frac onsite fraccing the 102H and 103H wells. At the time of my inspection, stage 6 on the 103H, had been perf'ed, gun was stuck at 14,477. Left at 12:20. No violations were noted during this inspection.
2650613 Primary Facility 2017-10-16 Routine/Complete Inspection No Violations Noted I entered the site and met with Anthony Murphy (Company Man). At the time of the inspection, the wireline was running in the Easton 102H (081-21684) on the 13th stage while the 12th stage was under pressure on the Easton 103H(081-21682). Field conductivity in puddles adjacent to the produced water storage containment was between 267-588 umhos/cm. Field conductivity in containment at the southeastern corner of containment was 356 umhos/cm. Dust was visible coming from the sand handling equipment. After looking into the sand concern, the filters were being cleaned before I left the site. The site was in excellent condition and as usual, the containment is graded toward the cellars and covered with rig mats. No violations noted.
2650644 Primary Facility 2017-10-20 Drilling/Alteration No Violations Noted Arrived at 14:40. Met with Anthony Murphy, company man, and Mike Cauley, frac engineer. At the time of my inspection, they were preparing to frac stage 18 on the 102H and 103H. They were also then going to frac stage 3 on the 29H. No violations were obaerved at the time of my inspection.
2654835 Primary Facility 2017-10-27 Routine/Complete Inspection No Violations Noted I met Justin Weaver when I arrived on-site. Frac was idle, and Justin informed me that they are only fracing at night. Individual secondary containments were in good condition, and rig mats on sitewide containment were clean and dry. Water trucks were offloading to the reused water tanks. Justin estimated that the frac would continue through the end of November.
2672649 Primary Facility 2017-12-19 Drilling/Alteration No Violations Noted Arrived at 11:50. Weather sunny, windy, temps in 40s. M35 with Anthony Murphy, company man Seneca. Currently drilling out plugs. Running 2 snubbing rigs (Hydraulic Pressure Control). At the time of my inspection, had just finished a well, changed out the bag, and were moving to the final well. No violations observed at the time of my inspection.
2676382 Primary Facility 2017-12-19 Site Restoration No Violations Noted The operator submitted a Post Drilling Well Site Restoration Report on 11/10/2017. Post Construction Stormwater Management (PCSM) plans have been implemented but vegetation is not sufficient to be considered permanently stabilized. Pipeline construction operations were ongoing at the time of the inspection.

After entering the site, I met with Anthony Murphy and we walked the site. One snubbing unit/workover rig was on the Easton 29H (081-21633) while the second was on the 101H (081-21683). The site was very clean and housekeeping was excellent. Individual containments were well placed and effective. Production equipment and flow lines were being installed as the site transitions toward flow-back/production. Overall, the site is in excellent condition but does not meet the requirements to be considered restored. No violations noted.

2682042 Primary Facility 2018-01-11 Drilling/Alteration No Violations Noted Arrived at 11:00. Met with Lee Kennemoth, completions foreman. Flow back operations are nearing a conclusion with 6 of the 7 wells on the pad are flowing gas to pipeline. Flow back ops anticipate another few days before turning site over to production. For the 101H, plugs still need to be drilled out.
2687981 Primary Facility 2018-01-29 Drilling/Alteration No Violations Noted Arrived at 10:00 Met with Glen Heckman (Seneca). This inspection took place as 6 of the 7 wells were being fully brought into production. For the 101H, plugs still need to be drilled out. For the 102 and 103, crew were preparing to shift production from Cameron's sand separators and gas buster for the water, to the Seneca GPUs. The other 4 wells were sending gas to the GPUs. Cameron will remain on site for some time. Left at 10:45.
2715428 Primary Facility 2018-04-04 Routine/Complete Inspection No Violations Noted Routine Inspection; 4/4/2018.

I met Aaron Hoffman (company man),and walked the site. At the time of the inspection, the wells were on temporary sand separation and produced water was being transferred from the production tanks to a battery of frac-tanks. Water on the well pad surface had field conductivity between 350-545 umhos/cm. Field conductivity at the south end of the frac-tank battery containment was 5,350 umhos/cm. I recommended (to Mr. Hoffman) that the frac-tank battery containment be vacuumed as soon as it is practical. The vac-truck was removing water from the well cellars and surrounding containment during the inspection. Water was ponded near the entrance to the site west of SugarCamp Road. I recommend evaluating the drainage in this existing swale between the two houses. No violations noted.

2746592 Primary Facility 2018-06-27 Site Restoration No Violations Noted Well Site Restoration Report Inspection; 6/27/2018.

At the time of the inspection the wells were in production and produced water was being stored in frac tanks on containment (a pump was installed and was actively pumping from the permanent production tanks to the frac tanks. HRI was onsite removing a temporary HDPE water line which had previously been buried in the pad. I recommend restoring the site in accordance with the ESCGP or requesting a 2-year extension. I recommend denying the Well Site Restoration Report. No violations noted.

2819086 Primary Facility 2018-12-26 Routine/Complete Inspection No Violations Noted Seven wells staged for production. Seven GPUs within 10 inch containment. No leaks detected from production equipment.

Service rig on location to run additional production pipe. Containment and rig matting in place. All baker tanks within containment. No violations noted.
