I walked the site and then discussed the inspection with Inflection Energy representatives. Perimeter E & S controls have been installed and are functioning properly. At the time of the inspection, significant earthmoving was underway. Timber had been cut and logs were being loaded and hauled offsite. Topsoil is being stripped and stockpiled throughout the site while the main cut and fill for the well site is just beginning. A major modification approved on 8/22/2017, expanded the footprint of Wetland A and trimmed back the footprint of the site by not clearing the area between Wetland A and Wetland E. This area between Wetland A and Wetland E was subsequently cleared. Gregg Saunders (Inflection) explained the clearing occurred inside the LOD and the area was cleared because significantly more topsoil was uncovered than expected. 2661089 Primary Facility 2017-11-14 Routine/Partial Inspection No Violations Noted Arrived at 11:09. This is a conductor-only inspection. The conductor was in place. There was no well cellar. The pad slopes were not stabilized. Construction equipment was onsite, but there were no personnel on site. The location would appear to be a target for enhanced erosion in the event of precipitation.
2715154 Primary Facility 2018-04-04 Routine/Complete Inspection No Violations Noted Routine Inspection; 4/4/2018.

At the time of the inspection, the the major earthmoving onsite was complete and the site was stabilized with straw mulch. Water was flowing from the cut-side of the tank pad, through the drainage features to the cut-side of the well pad. The water concentrating on the cut-side of the well pad is not conveyed to the infiltration basin. A sediment-laden seep originates on the fill slope of the well pad and discharges from the site to Waters of the Commonwealth. Sample 3440367 was collected at 11:30 from a pool south of the well site. I recommend meeting onsite to discuss the stormwater management and E&S control for the site since they don't appear to be functioning as designed. Violations cited to ESG29-081-17-0005.

2718081 Primary Facility 2018-04-09 Compliance Evaluation No Violations Noted Compliance evaluation inspection; 4/9/2018.

Dan Harvey (DEP) and I met with Gregg Saunders and Donald Stevenson (both Inflection). At the time of the inspection, top-rock was being hauled-in and applied to the tank pad. Top-rock has yet to be applied the the well pad and ditches have not been permanently stabilized. Water in the stormwater system was flowing east to west along the swale at the base of the cut-slope. The water was flowing clear and went subsurface in the northwest corner of the well pad. The seep documented in the Department's 4/4/2018 inspection report was not discharging sediment during the inspection. Violation cited on 4/4/2018 are considered closed. No testing has been completed to show connectivity between the stormwater system and the seep documented on 4/4/2018.

The Department plans to monitor the stormwater system as more permanent stabilization is established. No violations noted.
2780055 Primary Facility 2018-09-26 Routine/Complete Inspection No Violations Noted The Department (Brendon Logan) met with Gregg Saunders of Inflection Energy at 13:45. The site requires minor adjustments of the silt sock. The current silt sock follows the LOD and is creating a channel for water to be concentrated to a low point of the site. The plans will be redlined to show these changes. The site is on schedule for fall seeding and stabilization.
