I entered the site at 11:05 and walked back the access road. The site is cleared and the rough grading on the road has begun. The road is being constructed back toward the pad with geo-textile and limestone base rock. Perimeter E & S controls have been installed as depicted on the plans. I met Dale Weaver (DCNR Forester) onsite and we walked back the cleared access road. The UNT to Little Bear Creek is depicted on the plans and ends just upslope of the access road. Just west of the UNT to Little Bear Creek, a woods road on DCNR property crosses the access road. It is clear from the scouring that the woods road conveys water during rain events. I recommend conveying water across the access road and not concentrating the flow in the rock-lined ditch on the cut slope. No violations noted.

2627652 Primary Facility 2017-08-10 Preoperation Inspection No Violations Noted Pre-Operational inspection. The access road has been constructed from the beginning of the project (adjacent to the COP 726A pad) back to the well site. The slopes are seeded and stabilized. Much of the road extent is already well vegetated. The earthmoving on the pad is complete and the operator indicated top-rock would be applied from 8/11/2017 through next week. The folks onsite were surveying and staking for the infiltration basin. This inspection fulfills the Pre-Operational inspection requirement of the Oil & Gas Act as amended by Act 13. No violations noted.
2627659 Primary Facility 2017-08-15 Routine/Complete Inspection No Violations Noted We met onsite with PGE representatives to discuss two cross culverts near where the access road crosses a woods road about 2300 feet back the access road. The Department expressed concern that the cross culvert was not adequately installed to convey water across the access road. PGE representatives agreed to re-install the existing cross culvert at a lower inlet elevation to function properly. They also agreed to install a cross culvert where erosion from a pre-existing woods road was collecting material behind the clean rock of the french mattress. A Ziegenfuss Drilling Inc. rig was onsite drilling and setting conductor. Work continues to stabilize the site and add top-rock to the pad surface.No violations noted.
2627791 Primary Facility 2017-08-21 Routine/Complete Inspection No Violations Noted Arrived @12:58. Weather: sunny. Very long access road. Charles Construction workers were graArrived @12:58. Weather: sunny. Very long access road. Charles Construction workers were grading and compacting portions of the pad at the time of my inspection. This was a spud inspection. One well was spudded. The rectangular metal cellar was in place. The conductor was covered. Crew were preparing to backfill the backside of the cellar with gravel. No violations were observed at the time of my inspection. Left location @ 13:05.
2640821 Primary Facility 2017-09-27 Drilling/Alteration No Violations Noted Arrived at 13:50. Weather: hot and sunny. Nomac 359 on site drilling out surface casinf cement plug at 958', at the time of my inspection. Met with company man Chip Queen and engineer, Zac Fidurko. Surface casing set at 1044' KB. Haliburton pumped 490 sacks Bondcem. 45 bbl back to surface. Discussed centralizers: PGE uses 1 bowspring/ joint and uses double spring centralizers for inclinations > 8 degrees. Discussed casing integrity tests, WOC. There were no violations observed at the time of my inspection.
2643549 Primary Facility 2017-10-04 Drilling/Alteration No Violations Noted Arrived at 8:50. Weather: cool and sunny. Nomac 359 on site finished tangent and switched to SOBM, getting ready to start curve. At 8061' at the time of my inspection. Met with company man Chip Queen and engineer, Zac Fidurko. Reviewed IC cement job: casing set @ 2469.78 ground level. Surface casing set at 1044' KB. Haliburton pumped 69 bbl Halcem lead and 119 bbl Bondcem. 28 bbl return, 2 bbl bleedback. There were no violations observed at the time of my inspection.
2646237 Primary Facility 2017-10-11 Drilling/Alteration No Violations Noted Arrived at 15:20. Weather: warm, rain. Nomac 359 at 9045' at the time of my inspection. Met with company man Chip Queen and engineer, Zac Fidurko. On 10/7/17 found that Marcellus was approx. 150' higher than anticipated. Plugged back and kicked off the plug on 10/10/17. First plug from 9522' to 9018', second plug from 9018' to 8350'. There were no violations observed at the time of my inspection.
2649633 Primary Facility 2017-10-18 Routine/Complete Inspection No Violations Noted I met with Tony Harlan, Zac Fidurko, and Chip (no last name given) on-site. Nomac rig 359 active, running casing in the wellbore. Sitewide and secondary containments in good condition, although I noted a spot near the pipe rack where approximately 6" of foam berm appeared to be missing. Containment contractors will address this area. Chemical storage was clean, as was the cuttings backyard.
2650298 Primary Facility 2017-10-20 Drilling/Alteration No Violations Noted Arrived at 13:10. Weather: sunny. Met with Chip Queen, company man. Nomac 359 rigging down. Reviewed 5 1/2" cement report. TVD at 14826. Pumped 426 bbl lead cement (15.6 ppg 1.191 yd - 2010 sks). Rotated casing. Plan to move rig and star up at Butternut Grove towards end of next week. No violations were observed.
2769638 Primary Facility 2018-08-30 Routine/Complete Inspection No Violations Noted I met Matt Covine and Eric Duzick of PGE when I arrived. Wise Well Services coil tubing rig was on location, drilling out plugs. Containments and berms were being reused from the frac, but appeared to be in good condition. No water was present on or off containment. Housekeeping on the site was good as well.
2807535 Primary Facility 2018-11-28 Routine/Complete Inspection No Violations Noted One well staged for production/flowback. Flexsteel pipeline currently being run to the site. No violations noted.
