2656612 Primary Facility 2017-10-23 Preoperation Inspection No Violations Noted PreOperational Inspection. I entered the site and drove back the access road. Perimeter E & S controls are installed and functioning properly. Much of the access road and Mountain Meadow pad A have been stabilized. Top rock has been applied to the Mountain Meadow pad A while Mountain Meadow pad B is being graded and appears to be almost complete. Final seeding and stabilization of the site remains to be completed. PCSM controls have not yet been implemented but are planned next spring (according to representatives onsite). This inspection fulfills the Pre-Operational Inspection requirement of the Oil and Gas Act as amended by Act 13. No violations noted.
2710318 Primary Facility 2018-03-26 Routine/Complete Inspection No Violations Noted Weather was 35° and sunny at time of inspection. Well was covered with a steel plate and the celler was empty. Crews were in the process of laying containment on location. The rig is expected to move to this location on the upcoming weekend. No violations noted at time of inspection.
2716921 Primary Facility 2018-04-09 Routine/Complete Inspection No Violations Noted Routine inspection; 4/9/2018.

At the time of the inspection the H&P Flexrig 225 was onsite on the Mountain Meadow B 7H (081-21702) well. Schlumberger was onsite to cement surface casing. Overall, the containment was in good condition. No violations noted.

2723236 Primary Facility 2018-04-26 Routine/Complete Inspection No Violations Noted Weather was 48° and cloudy at time of inspection. Well was covered with containment at time of inspection. Well pad was being prepped for a fracturing company to rig up. No violations noted at time of inspection.
2725527 Primary Facility 2018-05-01 Routine/Complete Inspection No Violations Noted No activity on the site. When I arrived, I immediately noted a petroleum smell similar to diesel, and staining was visible on the sitewide liner. On the liner itself, I noted 2 small tears in the liner where staining was present on the liner. When I lifted the "ears" created by the tears, an oily residue was present on the gravel immediately below these spots. A larger tear that had staining on the liner did not appear to have any oily residue beneath it. I encountered both Rick Hall and Steve Barbier of ARD on the Brooks pad after this inspection; I told them about these spots, and they said the containment was being removed on 5/2 or 5/3, and they would assess these spots and remove soil and gravel as needed to clean it up.
2744745 Primary Facility 2018-06-20 Routine/Complete Inspection No Violations Noted No activity on the site when I arrived. The sitewide containment was still in place, as well as a gasbuster in secondary containment. Approximately 50-60 gallons of water was present in the gasbuster containment, showing conductivity of 11.4 mS/cm. A very large puddle (200 gallons or more) was present on the sitewide liner, running from the gasbuster containment to the west side of the liner; conductivity readings ranged from 10.8 mS/cm near the gasbuster to 1.5 mS/ cm on the west side. A smaller puddle (100 gallons or more) was present on the north side of the liner; conductivity reading of this water was .35 mS/cm. I recommended to Matt Buck, who I encountered on the Mountain Meadow Pad A, to have these puddles vacced. The pad itself was dry, with no indication any of the water on containment had been released.
2852559 Primary Facility 2019-03-11 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions. Sites were snow-covered at the time of inspection. One wellhead is present on each pad. Two GPUs are present within secondary containment on pad A. The GPUs and containment appear to be in good condition. A compressed natural gas (CNG) plant is present on Pad A. It does not yet appear to be operational. A gathering pipeline is under construction which will connect pads A and B. No production equipment is present on pad B at the time of this inspection. No E&S issues noted.
