2678791 Primary Facility 2018-01-04 Complaint Inspection Violation(s) Noted The operator called around noon on 1/4 to report a spill at the Westline 35 location. A PVC lead line cap fell off, allowing 2-3 barrels of crude oil to spill to the soil and ground around the well.

The operator immediately shut off the section of lease to the wells and began to place down pads and booms. Some oil on the surface ran off the site and over a bank onto a flat area below the site. It traveled approximately 100 feet from the source before stopping. Bi waterways were impacted.

The operator has hired a contractor to begin digging; they will be on site 1/5 in the morning.

2681423 Primary Facility 2018-01-10 Follow-up Inspection Outstanding Violations - Viols Req'd The operator has dug up impacted soil and removed from the site in roll off containers. They have removed 6 containers of contaminated soil and stored them nearby. Sampling was done by ER&R and the results are pending.
2686947 Primary Facility 2018-01-26 Follow-up Inspection Outstanding Violations - Viols Req'd The operator dug out soil on 1/25 after finding their first round of samples had some parameters above Statewide Health Standards. They removed two more roll off containers of soil. They were using a PID meter to monitor VOC levels of soil removed and to determine when to take the second round of confirmatory samples. Five samples were collected using the biased sampling method. Results are pending.

2700470 Primary Facility 2018-03-01 Complaint Inspection Violation(s) Noted This inspection was performed with WQS Brady Johnson following a report of a 2-3 barrel oil spill at the Westline 35 well location.

It appears as though a recent repair from a previous spill at this location failed during the freeze/thaw cycle and allowed oil to leak out into freshly disturbed soil. The product then flowed down over the bank in the same area it previously followed. Rain and thawing conditions allowed some oil to leak down over the flat area bank and travel approximately 150 feet from the spill site.

The operator had a crew including a mini excavator on site at the time of this inspection, and they dug two trench holes to collect oil and water and contain any fluid that might drain due to heavy expected rain/snow. Given conditions, they decided to bring a vacuum truck on site to pull fluid from the upper trench before rain intensified. The line at the well site was exposed and the valve near the split in the line was closed in order to prevent further contamination.
