Upon arrival at this location, it was discovered that a tree had fallen onto the pipeline riser located next to this well. The impact from the tree broke both the oil and gas gathering lines (two separate lines). There was approximately 5-10 gallons of crude oil on the ground at this time, however there was no crude oil discharging from the well or either of the gathering lines. Natural gas was discharging to the air from its gathering line. The source of this gas appears to have been other wells on the lease or sales line that are connected to the line.

I placed multiple absorbent pads on the oil that had accumulated on the location and also shut the well off/in. Upon reaching cell phone coverage, I contacted Gary Neely of D&S Energy and relayed details of the incident to him. Gary stated that he had recently been notified about the incident by a representative of the Allegheny National Forest (surface owner) and that a crew was currently on its way to shut the
