An inspection of the Allegany 900 pad in Allegany Township, Potter County was conducted on March 26, 2013. At the time of the inspection the pad was mostly snow covered. No puddles exhibiting elevated conductivity or erosion issues were observed at this time. No violations were cited during this inspection.

The 2HB well is producing, the Allegany 1 well was plugged in 2014, and the 2H well has not been drilled (permit will expire on 3/10/17). According to the routine E&S inspection log, the site was restored in spring of 2016. Permanent stabilization and post-construction stormwater management BMPs not observed due to snow cover. Containment structures at site were partially or nearly full with either snow or ice. Once the snow/ice melts, fluid should be recovered from the containment structures. No violations cited during the inspection. TLT

2643817 Primary Facility 2017-10-04 Routine/Complete Inspection No Violations Noted On 10/4/17 at 1238 hrs I conducted an inspection at the Allegany 900 well site. It was sunny and 70 degrees F. No one was present during my inspection.

The 2HB well has been drilled and is in production; the well was tagged. The Allegany 1 well has been plugged. Two poly production tanks are staged in containment structures; each held a minimal amount of water and field measured specific conductance was less than 500 micro Siemens/cm. No staining or evidence of spills was noted on the pad surface.

A small compressor is located at the north end of the well pad. A tote of waste oil was staged in containment in the compressor building. Routine containment inspections are being conducted; the last inspection was on 9/30/17.

According to the E&S inspection log, interim restoration was completed in 2016. The perimeter channels and sediment trap have been removed. The southern portion of the site has been stabilized and vegetation is becoming established. Areas that haven't achieved a uniform 70% vegetative cov

2651323 Primary Facility 2017-10-24 Administrative/File Review No Violations Noted On 10/24/17 I reviewed the Post-Drilling Site Restoration Report for the Allegany 900 well site. The Restoration Report was electronically submitted on 10/23/17.

I recently conducted a routine inspection of the site on 10/4/17. As documented in the inspection report, post-drilling restoration was completed in 2016 and the site is permanently stabilized. On 10/16/17, SWEPI Environmental Coordinator, Pat Bernethy, sent an email with photos to document that the silt sock had been removed upon achieving permanent stabilization.

The Restoration Report was approved on 10/24/17.

2652618 Primary Facility 2017-10-26 Site Restoration No Violations Noted A restoration inspection was completed at the SWEP LP Allegany 900 well site located in Allegany Township Potter County on October 26, 2017 at 1100 hrs. Due to conditions observed on site during this inspection, the notice of termination as recommended for approval at this time.

I departed the site @1245 hours.
