2186581 Primary Facility 2013-07-22 Routine/Partial Inspection No Violations Noted On July 22, 2013 an inspection was conducted at the Mitchell A 903 pad in West Branch Township, Potter County. The 4H (105-21623) and 5H (105-21624) wells are drilled. The 1H(105-21656) well is not drilled, but the permit is active. This permit is not listed on the site ID sign and should be added. The site is stable and well vegetated. No puddles screened exhibited elevate conductivity. The 1H (105-21620), 2H (105-21621), 3H105-21622, 6H (105-21625), 6H (105-21657), 2H (105-21741) and 3H(105-21742) have been reported as Operator Reported Not Drilled (ORND). No issues were noted and no violations were cited during this inspection.
2329818 Primary Facility 2014-11-21 Routine/Complete Inspection No Violations Noted
2444156 Primary Facility 2016-01-26 Routine/Complete Inspection No Violations Noted Routine inspection. SWEPI acquired wells from Ultra Resources Inc on 10/31/14. The 4H & 5H wells have been drilled and are equipped for production. Site is permanently stabilized. Impoundment was 3/4 full and frozen over. Site has not been restored. SWEPI intends to submit a request in 2016 to extend the restoration requirements for the site. No violations cited. TLT
2461496 Primary Facility 2016-03-01 Routine/Complete Inspection No Violations Noted
2470413 Primary Facility 2016-03-31 Routine/Partial Inspection No Violations Noted Partial inspection. DEP and SWEPI engineering and regulatory staff on site to discuss restoration plans. Some minor changes to original restoration plan discussed. A revised plan will be developed and submitted to reflect the changes discussed in the field. TLT
2478644 Primary Facility 2016-04-22 Routine/Partial Inspection No Violations Noted Partial inspection. Site was recently restored. Post construction stormwater management BMPs have been installed and disturbed areas are temporarily stabilized. TLT
2571705 Primary Facility 2017-03-15 Routine/Complete Inspection No Violations Noted
2624725 Primary Facility 2017-08-07 Site Restoration No Violations Noted On 8/7/17 at 1318 hrs I conducted an inspection of the Mitchell 903 well site. No one was present during the inspection. It was overcast and 62 degrees F.

I observed the site and reviewed the erosion and sedimentation (E&S) routine inspection log, the Notice of Termination (NOT) and the Well Site Restoration Report (WSRR).

The site consists of the well pad, production pad and a freshwater impoundment. Two wells, the Mitchell 903 4H and 5H have been drilled. According to DEP records, both wells are in production.

Produced water is stored in a production tank, which has built-in secondary containment (Photo #1). Puddles on the pad were tested for field measured specific conductance; readings ranged between 708 - 928 micro Siemens/cm.

The impoundment is located adjacent to the well pad. On 8/2/17, SWEPI submitted a Well Development Impoundment Restoration Request for the Mitchell impoundment; the request was forwarded to the Oil & Gas permitting section for review. The impoundment is protected with a fenc

2685688 Primary Facility 2018-01-22 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. The wellhead assembly all fittings and flow lines were tested for gas. Results of this testing was gas was detected on the 9 5/8" x5 1/2" vented annulus. No other gas was detected on the wellhead assembly or the well area.
2751338 Primary Facility 2018-07-12 Routine/Complete Inspection No Violations Noted I conducted a ROUTINE inspection of the SWEPI LP-Marshlands Play Mitchell A 903 Pad on 7/12/18 at 1330 hrs. The weather was 74 degrees, sunny and 38% humidity, winds NNE 5mph. No one was present during the inspection.

I observed the access road, well pad and freshwater impoundment. Two wells, the Mitchell 903 4H and 5H have been drilled. Both wells appear to be in production. Produced water is stored in a Highland production tank, which has built-in secondary containment. The tank was last inspected 7/6/2018.

The impoundment is located adjacent to the well pad. A protective fence and adequate freeboard were observed.

The well site has been restored and post-construction stormwater management (PCSM) BMPs have been installed. The ESCGP was terminated 8/7/2017.

No violations were cited during this inspection. I departed the site at 1430 hours

2773953 Primary Facility 2018-09-12 Routine/Complete Inspection No Violations Noted At the time of this inspection this well was in production. Valves on the production line leaving the wellhead assembly were open. A digital gauge on the well showed 309 psi (pounds per square inch) at this inspection. The Altair 5x multi gas meter reading was on a unidentified 2" diameter x approximately 2' in length open vented annulus. No violations noted at this inspection.
2831283 Primary Facility 2019-01-24 Routine/Complete Inspection No Violations Noted At the time of this inspection this well in production. A digital gauge on the well head assembly was showing a pressure of 420 psi (pounds per square inch). The valve on the production/flow line was in the open position. A single annulus vent that is the intermediate x production was tested for combustible gas with the results being 0% LEL (lower explosion limit) and 0% METHNE. No other annulus vents are vented to surface at this well. No violations noted at this inspection.
