2051258 Primary Facility 2012-03-13 Routine/Complete Inspection Pending A routine inspection was conducted at the Paul B 901 pad on March 13, 2012. The 3H well is drilled and the 6H has been spudded. AMEC conducted an inspection sometime between March 5 and March 12, 2012, the actual inspection date was not specified on the inspection report. The site ID sign was present, well drilling permits posted and the Erosion and Sedimentation (E&S) control plan was on site. All E&S BMPs were established and maintained. The fill slopes on the back of the pad will need to be monitored to ensure perennial vegetation is established to reduce the potential for accelerated erosion. A small puddle was observed near the steps to the production tank containment. The puddle exhibited elevated reading for specific conductance. A soil sample was collected and submitted for analysis. Violations may be cited pending the results of the sample. The area should be screened to determine the extent of the release and a plan to remediate the area should be submitted to the Department. - MRN

During a previous inspection conducted on March 13, 2012 (2051258) a small puddle was observed near the steps to the production tank containment. The puddle exhibited elevated reading for specific conductance. The n area has been excavated and soil samples collected by Ultra and submitted for analysis. During today's inspection, a puddle in the excavated area was screened with the conductivity fieled meter and a reading of 4.64µS was found. A reading of the fluid inside the secondary containment was taken and was found to be 7.28µS. Violations will remain pending at this time. No new violations were cited during this inspection. - MRN
2062519 Primary Facility 2012-04-26 Follow-up Inspection Pending An inspection was conducted at the Paul pad on April 26, 2012 at approximately 1345 to follow -up on the clean-up of a release discovered on March 13, 2012. During the inspection conducted on March 13, 2012 (2051258) a small puddle was observed near the steps to the production tank containment. The puddle exhibited elevated reading for specific conductance. The area has been excavated and soil samples collected by Ultra and submitted for analysis. During an inspection conducted on April 12, 2012 a puddle in the excavated area was screened with the conductivity field meter and a reading of 4.64µS was found. The results of the soil samples collected by Ultra indicated the presence of chloride at a level of 1,600mg/kg. On April 26, the day of this inspection, the area was excavated further and screened by AMEC using a conductivity probe. AMEC and DEP then collected soils samples from the bottom of the excavation area. - MRN

2153159 Primary Facility 2013-04-01 Routine/Partial Inspection No Violations Noted An inspection of the Paul 906 pad in West Branch Township, Potter County was conducted on April 1, 2013. There are six drill permits associated with this site. The 3H and 4H wells are on active status, the 1H, 2H and 5H wells have been reported not drilled by the operator and the 6H well has been placed on Regulatory Inactive Status. The 3H well is the only well in production. The pad was partially snow covered at the time of this inspection. What could be seen was either well vegetated or stabilized with stone. No puddles exhibiting elevated conductivity or erosion issues were observed at this time. No violations were cited during this inspection.

2748886 Primary Facility 2018-07-05 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the Marshlands Play-Paul 906 pad located in Westbranch Township, Potter County. The weather is 76°, mostly cloudy, humidity 86%, winds S at 8 mph. No activity was observed onsite. This site has had interim restoration completed. The pad size has been shrunken down and vegetation is growing. PCSM BMP's were installed in April 2016. Due to recent heavy rains standing water was observed on the pad surface. No violations were noted. I departed the site at 1215 hrs.

The valves on the 2" production line leaving the wellhead assembly were in the shut position. A gauge on the production line showed 0 psi (pounds per square inch). Four (4) more valves on the production line going into the production equipment on site all showed 0 psi. also. No violations were noted at this inspection.
