2000956 Primary Facility 2011-08-30 Compliance Evaluation No Violations Noted On Aug. 30, 2011 an inspection was conducted at the Coon Hollow 904 well site to determine if the violations noted on the 5/11/11 (1972553) had been resolved. The site ID sign, E&S plan, E&S controls and perimeter of the site were observed. The last routine inspection completed by AMEC was conducted on 8/30/11. Documentation that the affected area had been excavated and contaminated soil disposed of properly has been received by the Department and was deemed acceptable. The affected area has been backfilled following the clean-up and the photos document this. No new violations were noted during this inspection. An inspection of the 6H well permit was also conducted. It should be noted that the seeding completed in June 2011 has not established itself and warrants further monitoring.

Six wells have bben permitted, 2 wells drilled and one in in production. At the time of this inspection, the ground was frozen and partially snow covered. Slope blanket are in place and all slopes appear stable. No evidence of accelerated erosion was noted. No visible signs of spills or leaks were noted. No violations were cited during this inspection.

There are six drill permits associated with this site. The 3H and 5H wells are on active status, the 1H, 2H and 4H wells have been reported not drilled by the operator and the 6H well has been placed on Regulatory Inactive Status. The pad was partially snow covered at the time of this inspection. No puddles exhibiting elevated conductivity or erosion issues were observed at this time. No violations were cited during this inspection.

2624039 Primary Facility 2017-08-09 Site Restoration No Violations Noted On 8/9/17 at 0901 hrs I conducted an inspection of the Coon Hollow 904 well site. No one was present during the inspection. It was clear and 60 degrees F.

I observed the site and reviewed the erosion and sedimentation (E&S) routine inspection log, the Notice of Termination (NOT) and the Well Site Restoration Report (WSRR).

The site consists of the access road and well pad. Two wells, the Coon Hollow 904 5H and 6H, (Photo #2), have been drilled. According to DEP records, the 5H well is in production. The 6H well was placed on inactive status on 8/28/12.

Produced water is stored in a production tank, which is staged in secondary containment. Routine secondary containment inspections are being completed; the last inspection was on 8/5/17. Water in the containment structure had a field measured specific conductance reading of 447 micro Siemens/cm.

The well site has been restored and post-construction stormwater management (PCSM) BMPs have been installed (Photo #3 & 4). Temporary E&S controls have been remove

The wellhead assembly all fittings and flow lines were tested for gas. The results of this testing was that gas was detected on an annulus that vents the 9 5/8" x 5 1/2" production casing. No other gas was detected on the wellhead assembly or the well area.
2748964 Primary Facility 2018-07-05 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of the SWEPI LP Coon Hollow 904 well pad located in Westbranch Township, Potter County on July 5, 2018 at 1230 hrs. The weather was 76°, raining, winds South at 8 mph. No activity was observed on site. Interim restoration has been completed on the pad. The 5H well was observed in production. No volations are noted at this time

A digital gauge on the well head assembly was showing a pressure of 316 psi (pounds per square inch). The valve on the production/flow line was in the open position. A single annulus vent that is the intermediate x production was tested for combustible gas with the results being 100% LEL (lower explosion limit) and 17% METHNE. No other annulus vents are vented to surface at this well. No violations noted at this inspection.
