The site ID sign, E&S plan, E&S controls and perimeter of the site were observed. The access road and associated controls were installed and well maintained. The last self-inspection was conducted on 10/21/11. The slopes were stabilized. They had been hydro-seeded with some grass presently growing. Two conductors had been set and one well drilled. Bubbling was evident around the casing of the drilled well. Mike Panettieri, Oil and Gas Inspector was notified. No violations were cited during this inspection. - MRN |

The mailbox at this site could not be accessed. No violations were cited during this inspection. |

The status of the 2H well(105-21735) is Operator Reported Not Drilled (ORND), the 4H(105-21736) well is not drilled, but the permit is active and 6H (105-21737) is drilled to total depth but has not been completed. The Erosion and Sedimentation Control permit expires on October 31, 2015. The last inspection conducted on behalf of the operator was on April 3, 2014; no issues were noted. The perimeter erosion controls were installed and appear functional. Very little topsoil is present at the site; it is mostly rock. The site was sparsely vegetated, but stabilized. No active erosion was noted and no sediment transport was observed. The Department is requesting Triana Energy to submit a plan outlining their restoration schedule for this site as well as other sites operated by Triana. |
