2628086 Primary Facility 2017-08-16 Routine/Complete Inspection No Violations Noted On 8/16/17 at 0906 hrs I conducted an inspection of the Traub Pad A well site. No one was present during the inspection. It was mostly sunny and 60 degrees F.

I observed the site and the items maintained in the mailbox. The site consists of the access road and well pad. No wells have been drilled at the site. The Traub A-2H well permit (105-21837) was issued in 2014; it has been renewed twice and will expire on 8/26/17. A new well permit application for the Traub A 10U (105-21904) is currently under review; the well is listed as a conventional well.

The well site has not been restored and the pad remains at full size (300 feet x 400 feet). Post construction stormwater management (PCSM) BMPs are not installed. The site is permanently stabilized and Sediment Traps 1 and 2 remain in place.

Although an active well permit has been maintained, there has been minimal activity at the well site in the last several years which demonstrates an intent to drill. The ESCGP for the well site expired on 11/6/16. A Not
