Present were myself and Jason Logsdon, JKLM pipeline Supervisor. The weather was 8 degrees and partly sunny. This inspection was completed during a tour of JKLM's activity in the Sweden Valley area between 1100 hours and 1500 hours.

No violations are noted at this time.

2819850 Primary Facility 2019-04-02 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection conducted a routine inspection of the JKLM Sweden Valley 102 well pad located in Summitt Twp, Potter Co on 4/2/2019 @1115 hours. The weather was 42 degrees, overcast, winds S 14mph, humidity 30%. Onsite activity consists of a work-over rig and contractor, Deep Well Services fishing out the 2 3/8" pipe in the process of cleaning out and redrilling the 3H well. Scott West, JKLM was present onsite for the inspection.

Multiple issues were observed during the inspection and reported to onsite personnel. These issues were being corrected when i departed the site.

No violations are noted at this time. I departed the site @1320 hours.
