The weather was 72 degrees, light rain, Humidity 86%, winds SSW 6mph. JKLM is preparing to begin clearing for the HW 141 well pad and requested a Pre-construction meeting. John Watts, JL Watts (contractor) and Jeff Long, JKLM were present onsite.
The total site area is listed as 10.29 acres. A 500x350’ well pad, 725’ of access road and associated controls. The watershed is listed as UNT 22236 and UNT 22233 to Cushing Hollow (HQ-CWF) designation. An area of minimal tree clearing is located between the northern edge of the well pad and Sediment Trap ST-1. Mr. Watts stated they have no plans to clear this area and believe the basin and channels can be installed without disturbing it. A number of vernal pools and wetlands are identified on the E&S plan drawings. These have been clearly flagged and will also be marked with orange construction fence. No violations are noted |

The site is currently under construction, but no activity was currently underway. Due to recent precipitation, the site is very wet. The partially completed rocklined ditch had a roll of filter fabric at the top of the rocked section where installation of the rock terminated (see photo#1). Sediment laden stormwater from the site was observed flowing down the rock-lined channel along the access road, and discharging into the rock-lined channel along the Tom May Rd. This water then travels through a culvert, under the road, to a sediment trap on north side of the Tom May Rd. The trap was full of sediment laden water and aquatic vegetation. The trap was observed overflowing, discharging sediment laden water into the woods north of the Tom May Rd. This constitutes a violation of CSL 402(b) for potential to pollute waters of the C |

Operator is reminded the ECB should have good continuous contact with underlying soil throughout the entire length to be effective. ECB was observed sticking up over twigs and wrinkled up in some areas. Please reference standard construction detail#11-1 for proper installation of erosion control blankets. The same was observed for the TRM matting in the E&S channels. Please reference standard construction detail#6-1 vegetated channel, for proper installation of a temporary channel liner. Present for the inspection were JKLM representative Jeff Long and Earthmoving contractor John Watts, JL Watts.
The site consists of a short access road, well pad, and perimeter controls. The site is located in an HQ-CWF UNTs to Cushing Hollow and Cushing Hollow watershed. JKLM plans to spud and drill one unconventional well on the site, the 4V (API#105-21925). No violations are noted at this time. I departed the site @1140 hours. |
