Well staked not yet drilled. BMPs installed and functioning as outlined in ES plan. Previously disturbed areas appeared to be seeded and mulched with observed growth, although it is recommended that operator reseed or blanket soil pile in order to prevent a possible erosion event as minimal growth observed at this time. Red staining of soil observed at toe of pad slope¿operator investigated and believes decomposition from fill material is causing the red staining. Accompanied by WQS Marc Maestra during inspection.
2006180 Primary Facility 2011-09-28 Drilling/Alteration No Violations Noted At the time of inspection, no E&S or IW discharge issues at the site. Chevron contracted Paterson rig 255. Dewayne Pauley is the representative for Chevron. Conductor pipe was set at 60 ft. (20¿¿pipe) and was grouted in place. DEP performed a site walk and did not find any issues. Chevron/Patterson was in the process of procuring the site for drilling. According to Dewayne, drilling of the vertical section should begin during the week of October 2nd.

2009613 Primary Facility 2011-10-04 Routine/Complete Inspection No Violations Noted : Site visit was part of routine inspection due to spud notification. Inspection was conducted with April Weiland.
2012156 Primary Facility 2011-10-19 Drilling/Alteration No Violations Noted At the time of inspection, no E&S or IW discharge issues at the site. Chevron had contracted Paterson rig 255. Dewayne Pauley was present representing Chief under Chevron. Conductor pipe (20¿) was set at 62 feet and was grouted in place. The Surface pipe (13 3/8¿) was set at 417 feet and was cemented with 340 sacks of Type I with 34 bbls of return. The Intermediate pipe (9 5/8¿) was set at 1820 feet and was cemented with 600 sks of Type I with 20 bbls of return. Current depth was at 7620 feet. This drilling phase is projected to drill a vertical pilot hole to a depth of 8560 feet. Chief is anticipated completion time of 10/22/11 for this phase.
2017495 Primary Facility 2011-11-03 Routine/Complete Inspection No Violations Noted well sign, ES plan, ESCGP-1 present, but no PPC plan. Chevron Regulatory Compliance Specialist Alexander onsite. Access road stable and adequately maintained, wash rack located at site entrance. Pad adequately rocked. Well drilled, operator currently rigging down. Secondary containment located on pad beneath rig, as well as franc tanks. Bmp's installed according to plan and appear to be functioning, although department noted a piece of felt, plastic, geotextile, and various pieces of scrap wood place in ditches surrounding well site. Two unlabeled black barrels located off of secondary containment on wood pallets near soil pile. Department requested that operator place barrels on secondary containment as well as remove debris from ditches. Site has been seeded/mulched with minimal growth, recommend operator reseed and mulch soil pile to ensure adequate temporary stabilization is sustained. No accelerated erosion or sedimentation issues noted. Operator immediately responding to potential pollution concerns noted above.
2034155 Primary Facility 2012-01-10 Routine/Complete Inspection No Violations Noted At the time of inspection, well site ID sign, ES plan, ESCGP-1, PPC plan present. Well Spudded 9/21/11. Well site gated and locked. Access road stable. Pad adequately rocked. Well drilled not yet equipped for prodcution. Site appears temporarily stabilized. Temporary bmp's installed and functioning according to plan. No accelerated erosion or sedimenation issues noted. Since last inspection, soil pile has been reseeded. No violations noted during this inspection. Accompanied by WQS Marc Maestra.
