2462549 Primary Facility 2016-03-08 Drilling/Alteration No Violations Noted Weather: warm and sunny. The 1H was the only well on the pad. It was not in production. The 9-5/8” x 5-1/2” was the only annular vent, it was shut-in at the time of my inspection although plumbing to a vented polytank was on site.

On 3/7/16, observed bubbling in the well cellar. Contacted Jason Petlock and arranged to meet with staff on 3/8/16 to investigate. Arrived at 13:30, Mountain Energy vac truck was on site. Met with Mike Hronich (SWN). As cellar was vacc’ed out we could see that the bubbles on both sides were coming from leaking valves on the 9-5/8” x 5-1/2” casing vent, which was shut-in. Pressure on the vent was 170.7 psi, it blew down in a few seconds. The annulus was shut-in when I left the location. On 3/9/16 I contacted Afton Sterling (SWN) to discuss the rationale for allowing the casing to vent. She agreed to do this. No violations were noted at the time of my inspection. Left location @ 14:45.

Overall vegetative cover on site is greater than 70 percent. Temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

The drilled well on site is not in production, and no active drilling permits exist for the site. The Erosion and Sediment Control General Permit (ESCGP) was set to expire on 10/03/2015. On 10/1/2015, the Department issued a six-month extension which extended the permit expiration until 04/01/2016.

To maintain compliance with the Department's regulations pertaining to site restoration (58 Pa. C.S. § 3216), a 2-year restoration extension should be requested for this site.

No violations were cited during this inspection.

2534870 Primary Facility 2016-11-02 Routine/Complete Inspection No Violations Noted Weather: warm and sunny. Arrived at 11:10. The 1H was the only well on the pad. It was locked, tagged, and not in production. Cellar was filled with water, no bubbles. The 9-5/8” x 5-1/2” was the only annular vent, it plumbing to a vented 550-gal polytank was on site. The vent pipe entered the polytank at the base, below the water line. Bubbles were emitted approximately for a few seconds, once every 5 seconds. The concentration of combustible gas in the tank was 16%/volume (Altair 5X gas meter calibrated 11/2/16).

There were no violations noted at the time of this inspection. Left location @ 10:45.

2544499 Primary Facility 2016-12-13 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess site conditions. No Southwestern representatives were present.

The Erosion and Sediment Control General Permit (ESCGP) was expired on 04/01/2016. Currently, no active well permits exist for the site. To maintain compliance with the Department's regulations pertaining to site restoration (58 Pa. C.S. § 3216), a 2-year restoration extension should be requested for this site. Southwestern is reminded that a current ESCGP is required to conduct site restortation activities.

No violations were cited during this inspection.

2618893 Primary Facility 2017-07-25 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of SWN’s Castrogiovanni 1H conducted July 25, 2017 at approximately 1 pm. Crew was off location, well was shut in with master valve on b section. CIBP has been set in the 5 ½” production casing. Analog gauge showed ~650 PSI on the prod. csg.. Gauge for backside of production casing read 0 PSI, may be isolated at needle valve. I called the operator, they called back and confirmed the needle valve is just shut on the backside and the production casing gauge was just trapped pressure at the wellhead; production csg. is dead after setting CIBP. The rig plans to return after monitoring pressures on this well and finish the P&A

Gauge for backside of production casing read 0 PSI (isolated at needle valve.) I could now see the source of the bubbling was from a fitting on the well wellhead for the backside of production casing.

Bled off, gas, pressure built back up. Discussed w/ operator about setting additional CIBP at 8900' assuming first one failed. Rig pulled tbg, currently no pressure on casing. Operator suspects it may have been a trapped gas between first CIBP and wellhead rather than formation pressure, and will decide on setting the additional CIBP and cement or cement on top of first CIBP at this point. 2668561 Primary Facility 2017-12-06 Plugging(Includes Plugged/Mined Through) No Violations Noted Routine inspection of a plugged well. The well has been plugged, cut, capped, and buried below grade. At the time of inspection, there some standing water above the wells approximate location. No bubbling was observed. I used an altair 5x gas meter to monitor for gas in several spots at the wells surface location. No gas was detected. There were no obvious issues observed. Attached are photos from the inspection in addition to a photo of the marker cap before burial.
