The Erosion and Sediment Control General Permit 1 for this site expired on October 14, 2015. Southwestern has categorized this site as one needing a 2-year restoration extension. Currently, the Department requires extension requests to include post-construction stormwater management features that will be implemented in the interim until site restoration is accomplished. In this case, Southwestern is reminded that ESCGP-2 coverage will be necessary to conduct full site restoration.

2543403 Primary Facility 2016-12-09 Routine/Complete Inspection No Violations Noted Routine inspection conducted to assess overall site conditions. No Southwestern representatives were present.

Overall vegetative cover on the site is greater than 70% and temporary erosion and sediment control BMPs have been removed. No evidence of accelerated erosion or sedimentation was observed.

The Erosion and Sediment Control General Permit (ESCGP) 1 for this site expired on October 14, 2015. Southwestern has categorized this site as one needing a 2-year restoration extension; however, the Department has not yet received a request for an extension of time. In order to remain in complaince with the Department's regulations regarding site restoration (Title 25 Pa. Code § 78a.65, Title 58 Pa. C.S. § 3216), Southwestern should obtain a new drilling permit, request an extension of time for site restoration, or restore the site. If the site is restored, a new ESCGP would be required.

2600946 Primary Facility 2017-06-05 Routine/Complete Inspection No Violations Noted Inspection of the Tall Maples 5H (113-20078) conducted June 5, 2017 at approximately 10:20 am. The well had a production tree, is shut-in, and not tied into pipeline. There was an analog pressure gauge at tree top, isolated from well (0 PSI). The cellar was filled with water. No bubbling was observed. There were two pipes coming up from the cellar, shut in, no gauges. The operator intends to plug this well. No obvious issues were observed.
2605914 Primary Facility 2017-06-20 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of the Tall Maples 5H (113-20078) conducted June 20, 2017 at approximately 10:00 am. A rig is on location for plugging ops. The crew had iron rigged up to production tree, in process of pumping down well. Pressure before job was ~2800 PSI, reportedly no pressure on the backside of the production casing. CIBP will be set at 8850’ in the 5 ½” csg with CMT on top. The operator will log 5 ½” to determine TOC and see if cut and pull 5 1/2” csg is appropriate. No Issues.
2609157 Primary Facility 2017-06-29 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of the Tall Maples 5H (113-20078) conducted June 29, 2017 at approximately 10:00 am. A rig is on location for plugging ops. I spoke with the Companyman on location. The operator is waiting on Schlumberger who will gel uphole from plug inside 5 ½” back to the perfs in the 5 ½” at 4235’, where they will squeeze cement behind the 5 ½” to stop flow of gas. They were able to break circulation behind the 5 1/2" with brine, pumped ~3 bbl/min.After squeeze they will continue on to cut and pull 5 ½” and set remaining cement plugs to complete the P&A. No violations.
