2254196 Primary Facility 2014-03-17 Follow-up Inspection No Violations Noted A previous inspection (1/15/2014) found small piles of two substances on the pad. Both were promptly cleaned up, but there was some uncertainty about their identity. These proved to be a friction reducer and salt (see 2371 350 analysis attached). Before modifying the previous inspection report result from pending to violations immediately corrected, I conducted an additional follow-up inspection on 3/17/2014. The SW corner, where the piles were found was clean. It is worth remarking that, in the SE quadrant there were three small spots where a petroleum-based substance was on the pad. The diameter of each spot was ~10" -12". I would recommend that each of these spots be cleaned up off the pad. There were no other outstanding issues at the time of my inspection.

This inspection was prompted by a request for inactive status. There were one well and 3 water-filled cellars on the location. The well was not in production. Its cellar was water-filled, no bubbling. There was one visible annular vent. It was shut-in and not open to the gauge. There was no well tag at the well head. From the well record: Conductor: 40’, Surface casing: 639’, Intermediate casing: 2018’. The well was drilled to 10263’. It has been plugged with cement from TD back to 2118’.

Vegetative cover of the site is greater than 70%, and temporary erosion and sediment control BMPs are still in place. No signs of accelerated erosion or sedimentation were observed.

The Department's records indicate that no current well permits exist for the site, no extension request for site restoration has been submitted or approved, an no drilling permits are pending. According to the work plan for non-compliant well sites submitted by Southwestern on January 27, 2016, the wells on this pad are slated to be plugged and the well site restored sometime during 2017.

No violations were cited during this inspection.

2614923 Primary Facility 2017-07-11 Plugging(Includes Plugged/Mined Through) No Violations Noted Inspection of the Bohensky 2H conducted July 11 2017 at 3:15 pm. A rig is on location for plugging ops. They tagged bottom plug in openhole and circulated oil base mud out the hole with gel. Schlumberger is on location. They pumped cement plug across 9 5/8” shoe (2100’-1900’), displaced, and the rig pulled tubing out of balanced plug. No issues were observed.
