2275053 | Primary Facility | 2014-05-29 | Preoperation Inspection | No Violations Noted | The access road and associated BMPs were completed. The perimeter of the site was inspected and the silt sock had been installed as per the E&S plan and appeared to be functioning. There was a gap in the silt sock along the southeast perimeter of the Limit of Disturbance, near Sediment Trap 2 that present a potential to discharge sediment to an identified wetland area. The super-silt fence baffles in the Sediment Traps were improperly installed. As per the E&S plan and the E&S BMP manual (see Standard Construction Detail # 4-10), the fabric toe should be placed in the bottom of the trench, backfilled and compacted. The toe of the fabric of the super silt fences in both sediment traps was laid on the ground surface and cover with soil. This constitutes a violation of § 102.11(a). This violation was cited against the 113-20255 permit and was immediately corrected. No violations were cited against the 113-20260 or ESG29-113-14-0001. |

2421264 | Primary Facility | 2015-11-02 | Routine/Complete Inspection | No Violations Noted | Arrived on site at 13:00 and departed at 13:30. Weather conditions were partly cloudy ~60°F, with recent significant rainfall. No Chief representatives were present during the inspection. |
2427795 | Primary Facility | 2015-11-23 | Routine/Complete Inspection | No Violations Noted | Arrived @ 12:50. Weather sunny, cold and windy. There are three producing wells on location. Met with Jon Hafer (Chief) to check on a Chief report of combustible gas on the 13-3/8” x 9-5/8” annulus of the WL SE Unit 1H. We also checked on the 9-5/8” x 5-1/2” which had 100% methane (as detected with an Altair 5X gas meter calibrated on 11/23/15). Gas could be heard bubbling through fluid in the 9-5/8” x 5-1/2” annulus. Hafer isolated the 13-3/8” x 9-5/8” annulus by opening the hammer union connecting the plumbing of the annular vents. After isolation, the 13-3/8” x 9-5/8” annulus did not show any combustible gas. It was also noteworthy that the small check valve (a gate valve) located in the 1” pipe for the 13-3/8” x 9-5/8” annulus was not properly aligned. It is plausible that the misaligned valve was allowing combustible gas from the 9-5/8” x 5-1/2” annulus to the 13-3/8” x 9-5/8” annulus. No violations were noted at the time of inspection. Left location @ 14:13. |
2496281 | Primary Facility | 2016-06-27 | Routine/Complete Inspection | No Violations Noted | Routine inspection conducted to assess overall site conditions. No Chief representatives were present.
Vegetative cover on site is greater than 70% in some areas, but less in others. Temporary erosion and sediment control BMPs are in place and in good repair. No evidence of accelerated erosion or sedimentation was observed. According to the Department's records, a request for a 2-year extension of time for site restoration was received on 03/16/2016. As of the date of my inspection, this request has not yet been approved. No violations were cited during this inspection. |
2556812 | Primary Facility | 2017-01-30 | Routine/Complete Inspection | No Violations Noted | Routine inspection to assess overall site conditions. No Chief representatives were present during the inspection.
Vegetative cover on site is greater than 70% in most places though some areas were obscured by snow cover, and temporary erosion and sediment control BMPs are in place. No evidence of accelerated erosion or sedimentation was observed. This site was due for restoration in November 2015. According to the restoration schedule submitted by Chief, this site is anticipated to be restored during Spring 2018. While this proposed schedule has been acknowledged by the Department, Chief should still submit a request for a 2-year extension of time to restore the site in accordance with Title 58 Pa. C.S. § 3216 (relating to site restoration) (2012 Oil and Gas Act). No violations were cited during this inspection. |
2682051 | Primary Facility | 2018-01-12 | Routine/Complete Inspection | No Violations Noted | Three wells in production phase. Three each sand and water separators within containment. Natural gas powered suction engine on location. No leaks detected from production equipment. No violations noted. |
2811587 | Primary Facility | 2018-12-04 | Routine/Complete Inspection | No Violations Noted | Arrived onsite for a routine inspection at 12:21; the weather was ~30°F, and calm.
There were 3 properly tagged Marcellus wells staged for production. All the well cellars were filled in with gravel. There were 3 GPUs, 3 produced water tanks, 1 compressor, and 1 freshwater tank onsite. Two infiltration basins were onsite as PCSM. Both basins contained a fair amount of ice and snow. The rest of the area was free from snow and was greater than 70% vegetative cover. No E&S issues were observed at the time of inspection. According to Department records the ESCGP for this pad expires on 3/23/19. It also appears that a 2-year site restoration extension was acknowledged on 10/5/18. No violations were noted at the time of inspection. |
