There were three wells at this location: The Hurley 1H, a producing well was shut-in. The Hurley 2H and 3H were TD’ed. I was accompanied by Jon Hafer (Chief), who also brought a fresh water truck, and a vac truck. The purpose of this inspection was to fill the cellars of the Hurley 2H and 3H with enough water to determine whether there was methane outside of surface casing. Water was added to each cellar to a level just above the wellhead flutes, where the cement between the conductor and surface casing was exposed. Following the addition of the water, we waited approximately 15 minutes and searched for any signs of bubbling. No signs of bubbling were seen in the cellars of the Hurley 2H and Hurley 3H. Following these observations, the vac truck removed the water from the cellars. The result of this inspection for the Hurly 2H and the Hurley 3H is NOVIO, because no violations were observed during this inspection. Left location @ 11:45. |

The pad has been partially reduced in sized and overall is >70% vegetative cover. All PCSM BMPs appeared to be installed and functioning as designed. There were some small areas lacking in vegetation but were stabilized with straw and netting. Compost filter sock (CFS) remained along the access road and surrounding the pad including the revegetated areas. At the entrance of the access road there was a low spot which was retaining water(Photo #9). There are 3 properly tagged Marcellus wells staged for production. There are 3 GPUs and 3 produced water tanks onsite and in secondary containment. The thief hatch on the 1H tank appeared to be open at the time of inspection (Photo #6). A routine inspection was recently conducted on 10/11/18, ID# 2788086. PCSM had been observed during this inspection. The site was well vegetated with the exception of some spots that were stabilized with straw and erosion control matting. During the inspection on 10/26/18 the straw covered areas showed some new vegetation growing. The pad size was previously reduced and the site was >70% vegetative cover. No E&S issues were noted during the inspection. However, compost filter sock (CFS) remained around most of the pad and along sections of the access road. Temporary E&S controls should be removed as part of site restoration. The Department recommends returning the NOT and resubmitting it after the CFS and any other materials not part of PCSM have been removed. No violations noted. |

During this inspection it was noted that PCSM had been addressed, the site appeared stable, and the site was greater than 70% vegetative cover. The NOT was returned based on compost filter sock (CFS) remaining in place. The CFS has since been removed and the contents spread over an area to serve as a soil amendment.
